Were I in charge of operations, I would assign one MT (senior and highly skillled) only for each doctor as the primary editor and only his/her transcription would affect the speech engine. Other MTs would, of course, transcribe the dictations as needed, but their efforts simply be returned to the client but never affect the coding. The MT specifically assigned to that physician would also do all of the QA for that physician, and update the speech engine as needed.
This would ultimately ensure accurate speech coding for each dictator.
But the people running these companies these days don't know much about medical transcription, I suspect.
