Your contract will indicate the details of what they actually pay for regarding the definition of a line. They do not pay for headings, footers, signature lines, or anything in the demographics, although you are responsible for many details in the demographics which you do not input but need to verify and will be deducted an entire report's pay for an error.
Yes, as like any other company, they can manipulate the system of payment and they do, always to their advantage. What and how they charge the client is not the same as how they pay the MTs and speech editors.
ALL of their work is heavy ESL. Trust me, you cannot negotiate for another account and most of the time not even for additional accounts if you do not have enough work to keep you appropriately busy for your designated shift. As an IC, you will, however, be required to work a shift and produce a specific minimum of lines per shift.
If you checked out their website, you will notice that this company has other interests more heavily involved in the selling of platforms and coding and data mining, not so much in the delivery of medical records. They have less than half the MTs and SRs today than they did about three to five years ago, and this has nothing to do with being able to use their SR system twice as fast as traditional transcription. The workload for this part of their business has declined.
This information is all based on my observations.
