The old ASR actually kind of made sense of what was picked up versus what the doctors said but now it just inserts things at random that is not even close to what the doctors are saying, like in reports I had this week it was inserting diagnoses that the doctors never said or leaving off dictation that was said, etc. Its gotten worse, as we all know, not better. I honestly think I'd rather have it in its earlier stages than recently when it is bizarre in what it spews out. Had "ginger ale" for genitourinary the other day. Clear speaking doctor too.
