to dictate that perhaps he asked someone how to do it, and someone wrote down all the numbers to punch in (for that particular patient), and so he thought those were the numbers to get into the system each time. Years ago, a doctor asked me what all the numbers meant that he punched into the system. He had no idea that they were patient identifiers, so he stopped typing in random numbers (and THEY get the big bucks?!).
