report this dictator who routinely enters this wrong info just so the facility can be made aware and hopefully nip it, but I wouldn't count on it. You were "chewed out" because not catching it led to the report hitting the wrong chart. The physician entering the info who is just plain lazy isn't posting the info the patient's chart directly. You are the buffer and expected to catch the error unfortunately. We're all in the same boat and it stinks, I know.
