This could be extremely expensive. Docs don't use cassette tapes anymore, so whatever you purchase in the way of a way to transcribe the reports, would have to be compatible with the doc's system. It would be your expense, for sure.

Not trying to burst your bubble, but going around to clinics (I'm assuming you're talking about clinic work and not acute care) to get work to work from home is getting harder and harder---many don't even hire transcriptionists anymore, period. In fact, most don't.

SO, I would investigate further before worrying about HIPAA. One step at a time!
