Hospitals were upset at the number of blanks, so MM even had a contest for most improved decrease in blanks. Didn't change my behavior. I leave blanks for a reason! And, I leave a lot more blanks with this idiotic 99.65% requirement. One thing that frustrates me, though, is when QA fills in a blank that I am positive they only filled in with what the doctor usually says and is pretty standard. When I did that, I was punished for that, and the "corrected" version was basically the same regarding the content.
