They got the benefit already when they did not have to dictate it.

You might also remind them that use of the same report over and over, especially in EHRs, is getting a lot of attention right now for patient safety and billing fraud reasons.

Let's say that on visit #1, they did a big, long note with a full history, PE, and all. On visit #2, you use that same note, updating a few bits in the HPI and plan, but in actuality, the doctor did NOT do that full PE. Same thing on visits #3 and #4. If the doctor bills for a higher level of service based on all that included text that did not happen, it is fraud. He can only bill for the bits he did, and that is all that should be in the report.

You don't have to do a full history, ROS, and PE at every visit. Even if doctors do it, it is not necessary. Medicare knows it, too, and so do other insurers.

Your guy is not only trying to cheat you, he is cheating the patient and their insurance.

On the bright side, you can figure out what he is up to and file a whistleblower complaint.
