Yes, Efreeka when it should be African is easy compared to others. However, it illustrates how pronunciation can be all wrong. The example you give in which the correct word is "hypothyroidism" couldn't possibly be figured out with 100% certitude to its accuracy, IMO, unless you refer to previous reports on patient. Medical reports need 100% certitude as to what speaker is saying and not guessing. That's why ESL with heavy accent or mumbling, slurring speakers shouldn't be allowed to dictate.
