I recently lost a long-term in-house job and started up again as an IC. I did it years ago but at that time picked up tapes, printed and delivered. Now, I do it all electronically. When I got my first client, I simply told the office manager that I don't do tapes with pick-up/delivery anymore and am only accepting digital files. I have Express Scribe professional version, which was in the $40 range I think. It plays multiple file types. The office manager bought Sony handheld digital recorders that records in MP3 format. I asked them to check with me before he decided whatever they were going to purchase so I could make sure the file format was listed as one of those supported by my Express Scribe software. It works beautifully. They record the dictation, upload to PC via a dock or cable (not sure which) that came with the recorders, encrypt the files with a program called MEO and email them to me. I then type everything up in Word, encrypt the files with MEO and email back to them. I am not fond of MEO honestly. There is another option for encryption, Sendinc.com. The only problem is the file size on the free version is limited to 10 mb I think, but a subscription can be purchased (on the doctor's end) for about $4 a month. They did not want to go this route at this particular office.

I felt funny about something when I was looking for the work -- telling someone I want your business, but it will require you to buy equipment. However, having been out of work, I simply could not provide the handheld recorders. I provided the Express Scribe, pedal, etc. that I need to transcribe with, but they provided the digital recorders and MEO (license for them and me). I was not willing to pay for a license for MEO since sendinc is a free option, as long as they make sure to record smaller files instead of one huge one. They chose this way so the doctors would not have to worry about keeping the files to a certain size. So, I work with their program that they provided to me even though I like it less than sendinc.

I used to have a call-in system many years ago, but I got rid of it when I began working in house. It doesn't pay to have it anymore in my opinion because most offices have some sort of electronic system in place, and whatever system I were to buy may or may not work with what they have.

I LOVE the way this is working out. There is no recurring expense, and I have a VPN service on my computer for added security (I provide this because I like it anyway just for my own internet security), and all I need to do is find a wireless connection (I can work in hotels, etc.) and I can do my work from anywhere in the world.

I hope this helps some. I think your biggest challenge is finding the clients, but as someone said below, there indeed still exists some physicians who want to dictate.
