Nuance ambulatory MTs & FIESA - BBMT

I work with the ambulatory team (ortho clinic notes). I have 2 exclusive dictators and 2 that are shared with a couple other MTs. I was originally with On~Time Transcription. We were acquired by Accentus, and Accentus was acquired by Nuance a short time later. The Nuance acquisition took place in November 2012 I believe, and we were switched from IC to employees earlier this year in April.

I’m not completely clear on what FIESA is and how it works, but from what I have read it isn’t good.  I am not currently using FIESA, and there has never been any mention of it at all. I’m not sure if only certain platforms use it, but I’m using Emdat Inscribe, which is the platform I have always used (even back with On~Time).

I just watched the new Quality 2.0 video, and it mentions how you can view this new score in FIESA. I have never been given log in info or any info at all for FIESA. I do get occasional spot checks, but my manager just emails me a Word doc with a couple reports that have been audited. I get them every couple months or so. I have gotten 4 this year so far. For those of you who have come to Nuance by a similar path as me, are you using FIESA to view audited reports? Has there been any mention of using it as some point in the future?

QC and postdelivery QA auditing. Basically, anything you pend to QA will be run through FIESA. Errors, etc. will be deducted. The scoring on predelivery is supposed to be for "educational purposes" and not have any effect on bonus, etc. Post delivery audit are done on reports sent to the customer without pending to QC. These will be done continually. These scores count/determine bonus eligibility, etc. Be prepared to feel beaten down. Everything you read on here is correct. FIESA will make you feel as though you are brand new to the job. Making matters worse, you have workers in India doing FIESA on your reports because it's cheaper. I know a couple weeks back there was an ambulatory MLS on here boasting her $20 hour awesome job on ambulatory side. Everyone said wait for FIESA but he/she apparently felt that wasn't a consideration on that side. It's coming and you'll know when it does. Good luck! Thankfully I'm only a year from retirement and I'll be done with this mess.

Thank you for your reply. I also make a decent wage at the moment, but I don’t kid myself about what’s up. From the time of the acquisition I have lived in a perpetual state of waiting for the other shoe to drop. They don’t usually do everything all at once with these things. For example, the mandatory conversion to employee statusâ€Â¦ Nuance took over in November 2012, and that employee conversion didn’t take place until April 2014. The changes come slowly but surely.

On this side (ambulatory), at the moment we aren't even given any sort of possibility of getting a bonus. I got the email about viewing the Quality 2.0 thing in Enrich and just watched it today. It looks like they changed the scoring points for errors (more points than before). Hmmmmâ€Â¦ wonder why they did that? lol. I figured maybe they might be bringing us ambulatory folks into the whole bonus loop/FIESA thing soon. That’s a lot more possible bonuses, so of course they are going to make it even more impossible to achieve those bonuses.

I have 3 years and 5 months before my house is paid off, and I’m going to go manage the office at our auto repair shop. We need the extra (separate) income at the moment though, so for now I’m stuck here. I’m only hoping I can stick it out here and keep making a half-way decent wage until then. I have a feeling I’m going to need to keep an eye out for those dropping shoes thoughâ€Â¦ wouldn't want to get smacked upside the head with one, lol.

On the Ambulatory side, how do we find out what our QA% is?  Is it supposed to be in FIESA?  The only report I see in FIESA is from 11/8 to 11/14 payweek and it says Accuracy 100%.  There are no reports for any other pay week. Also, how do we find out what percentage we sent to QA?  I know under the new plan if we send more thn 10% to QA we get a reduced line rate, but I have no idea how much I send to QA.  Help please? ...

I was just wondering if anyone posts here that work for the Nuance Ambulatory Care side of things.  All I see here are what appears to be posts from the acute care side and they are not very favorable.  I will probably get hung on this board for saying this, but so far things are pretty good on this side of the fence.  Same managers, same good accounts, plenty of work.  Flexible schedule.  Not crazy about all the logging in to everything (WorldClient, WebClock, Spark), b ...

Now I understand how there were glowing reports of being happy and making good money.  Welcome to the way acute care works ambulatory folks.  We have a pool of accounts and reports come in with the one closest to turnaroundtime first. Ambulatory is definitely different than acute transcription and probably better in a LOT of ways, but if it makes you acute care feel any better, the 24-hour TAT that is talked about was not a true 24-hour TAT, at least not with my company. If my doctor dictated at 11 pm, my reports were due by 3 pm the next day, so that is more like a 16 hour TAT. If my doctors decided to turn in their reports on the weekend, then I transcribed on the weekend. I worked when my doctors sent reports which ...

Does anyone actually make their line count on the above?  The software is horrible.  Each patient is like 10 to 15 lines on average.  You would have to do 130 patients a day transcribing, is that possible?  And the speech rec, OMG that is horrible to try to make lines with.  And, they are taking 50 lines per 1000 away because it is an easy platform.  Yes, I checked with the Department of Labor and they can get away with it.  Crap, I have to wait 5 seconds when ...
