HOW IS AMBULATORY DOING? OOW? STILL HAVE EXCLUSIVES? - me

Ambulatory has been pretty quite here lately. We are curious...how is the transitioning going? Do you guys still have your exclusive doctors and are you able to make the same amount of money? Out of work situations often? How have things changed/stayed the same?

I'm not happy at all. I think some people have kept their exclusives, but others (like myself) have new managers who weren't originally with the On-Time/Zylomed/Accentus group (which is where the exclusive thing originated), and they are not comfortable with people having exclusives. They don't know how awesome it is for the MLS *AND* the client to have one MLS typing the same dictators day after day. The client gets exactly what they want, and it has a wonderful effect on the MLS's pocketbook, lol.

Personally, my weekly wages went from an average of $17-18/hr to an average of $8-9/hr. It plummeted once all the accounts were pooled, and I had to try to get used to multiple new dictators spread over multiple different accounts with so many new specs to try to remember that it made my head hurt. There are a few absolutely terrible dictators in there as well, and when they hit my queue it can go as low as $5-6/hr while I'm typing those shits. Oh.. and there's also the nifty schedules we had to choose as well... didn't have those before either.

Goodness, I certainly can't leave out working on the holidays! That's somethin' I'm not used to. I had always worked as an IC up until Nuance forced us to convert over to employees back in April, so this is the first Thanksgiving I've ever worked during my MT career. That was pretty neat. Not much work there, but the work that was there was a bunch of crap from the day before that nobody wanted to do, I guess because the dictators were horrible. I also had never laid ears on them before, so I think I made around $23 yesterday, and that took around 4 hours... then the work ran out. Did I mention how unbelievably horrible the dictators were? lol. I'm pretty sure the one guy dictated all of his reports without parting his lips once. He just hummed it. "Hmm hmmm mmmph mph mph hmm hmmmmm." What? What is that? Is that Katy Perry's new song? You should open your mouth a little wider when you speak.

Truth be told, I have my resignation letter all typed up and ready to go. Not sure when I'm going to actually send it, but I predict it could very well be before the end of this year. I was going to try to stick it out for around 3 more years, but I really don't see that happening.

I honestly don't mind having a schedule, being an employee, or even working on the damn holidays. That stuff wouldn't make me quit, but I've always said that when the day came that I was literally miserable sitting down to work everyday and not making any money... I'm outta here. That day is upon me. I'm pretty miserable... and pretty broke too.

If I do resign, I won't do MT anymore. I'll find something else to do... anything else! I have no desire to start from scratch with a new company. It's too iffy. MT is like a box of Whitman's Sampler assorted chocolates. You never know what you're gonna get, but most likely it will be something horrible that you hate. There are only a few yummy caramels in that box, and most likely someone else already got 'em. I'm not taking the chance. I'm just going to look for a better box of chocolates where the odds are a little better that I'll get something tasty that I can really sink my teeth into. ;o)

They know and we know they don't need all those people. They just want to ruin the holiday for us. Well MISSION ACCOMPLISHED. And this is why you need to have a constant ad running. People that don't HAVE to work for you DON'T. To put that in language Nuance management can understand: "Nobody likes you because of your ridiculous policies. You have a foul effect. (Did I use those right?) People leave you because you don't treat them well."

I agree. Some of us didn't even realize we had to work Thanksgiving until Wednesday! It's not about the work, the schedule, or trying to help clear things. As of 11/12, we were told we could keep our exclusives, but if we didn't have the 4 hr TAT, it would be opened up. Ever since then, it's ALL been pool. I had 1 exclusive. That was my income. I made $1 per hour last week, $2 per hour last week, and Wednesday, I made a whopping $2.50 per hour. My check didn't just decrease by half, but it has decreased 75%. And it's almost Christmas. You get them talking out of both sides of their mouth (exclusive vs pool), and the quality is definitely going down when you spend 10 minutes on a 50 second report and have 10 blanks in it! If this is for the better, I don't know who it's for the better of....unless it's the higher ups who are still making money bleeding us to death. I know there are a few managers still who are trying to help their MTs, but there are some who are just in it to keep their jobs, and whatever they have to do, they are going to do. I've never made this less in 20 years. I've never been this miserable in 20 years. And after years of loyalty, working any hours asked, any days asked, any holidays asked, helping whenever I could, I now am perusing the want ads. It's a shame that someone of Nuance's magnitude doesn't value those who make them the money. But then I guess that's how a lot of companies do business. When you take the ones who know the accounts and put someone who has never even done that specialty on, you know there are going to be mistakes. I can't imagine it won't take long for the companies to notice. Maybe the grand design is for them to be transferred to Dragon. After all, Nuance made that too! OR, if we make mistakes, they can complain that it's not them and send it to India. There seems to be some grand design we don't know about, and I keep holding out, but it seems to be getting worse and worse. People have families to take care of, and I don't know ANY MT at this point that isn't suffering, financially and also emotionally, with this. I say shame on Nuance. I'm making LESS than minimum wage, and that's not right. Loyalty should account for something!!!! Nuance obviously doesn't care about it.

Yes, they were crappy. It was also specialties/accounts I wasn't familiar with. So I spent at least 15 minutes reviewing specs, time spent looking up old reports, researching terminology, and as soon as I finished one, I had a different clinic and went back to reviewing specs, looking at old reports....after 3 hours, I literally made about $8. Very defeating.

Several months ago I posted here about how wonderful Ambulatory Care had it. We had it all: Exclusive accounts with good dictators, no weekends, no holidays, and plenty of work. I was nicknamed Princess after that post and many said "just wait and see what happens down the road." Well, I am here to tell you all that you were RIGHT! I am also here to apologize for being so confident and also lacking compassion (which was really not my intention) for my fellow MTs on the acute care side. When "the changes" started occurring back in the fall, I started looking, and I mean looking hard. It paid off and I found a good small caring company to work for. I left Nuance, giving them a notice of the length of time it took me to write and send them an email, telling them just how happy I was going to be when I left them in the dust, and then did just that--LEFT then and there! There are still good companies out there. They are hard to find at times because no one who works for them ever leaves. But keep looking if you still want to be in this profession. I tested and even tried out a few places before I found the one for me. I wish all of you in acute care and ambulatory care, the best of luck in getting away from this MT company from hell, and again, I apologize for my overly positive/cocky attitude months before. I have read posts on this board over the past few months that have brought me to tears. I pray for all of you who are suffering so mentally and financially as you try to desperately hang on and keep going here. Have faith and keep looking!

Just speaking for myself because it seems the situation is different depending on which TSMs you are under. I got my exclusives back with a 12 hour TAT.

As far as being OOW, I personally have not been. My exclusives keep me busy for most, if not all, of the day, but when they are done, there is usually some work in the pool.

I didn't realize we had to work on Thanksgiving either, but luckily there was no dictation Thursday or Friday, so it was a nice little holiday for me. I really needed the break.

I got the e-mail about the new compensation plan, so I'm a bit worried about that. They say the TSMs will be calling meetings with us this week to explain the details.

Ambulatory is definitely different than acute transcription and probably better in a LOT of ways, but if it makes you acute care feel any better, the 24-hour TAT that is talked about was not a true 24-hour TAT, at least not with my company. If my doctor dictated at 11 pm, my reports were due by 3 pm the next day, so that is more like a 16 hour TAT. If my doctors decided to turn in their reports on the weekend, then I transcribed on the weekend. I worked when my doctors sent reports which ...

Kiss your exclusives goodbye.  As of right now, I am seeing other people's exclusives and they are seeing mine in their pool as well. The way it works is your exclusives will feed in at the top with higher priority, and then after those, you will see other people's exclusives feeding in at lower priority. So, if someone's exclusives don't dictate one day, or they only dictate a little bit, then that MLS will be all over YOUR exclusives. Yay.  So excited. ...

On the Ambulatory side, how do we find out what our QA% is?  Is it supposed to be in FIESA?  The only report I see in FIESA is from 11/8 to 11/14 payweek and it says Accuracy 100%.  There are no reports for any other pay week. Also, how do we find out what percentage we sent to QA?  I know under the new plan if we send more thn 10% to QA we get a reduced line rate, but I have no idea how much I send to QA.  Help please? ...

I was just wondering if anyone posts here that work for the Nuance Ambulatory Care side of things.  All I see here are what appears to be posts from the acute care side and they are not very favorable.  I will probably get hung on this board for saying this, but so far things are pretty good on this side of the fence.  Same managers, same good accounts, plenty of work.  Flexible schedule.  Not crazy about all the logging in to everything (WorldClient, WebClock, Spark), b ...

I work with the ambulatory team (ortho clinic notes). I have 2 exclusive dictators and 2 that are shared with a couple other MTs. I was originally with On~Time Transcription. We were acquired by Accentus, and Accentus was acquired by Nuance a short time later. Does anyone actually make their line count on the above?  The software is horrible.  Each patient is like 10 to 15 lines on average.  You would have to do 130 patients a day transcribing, is that possible?  And the speech rec, OMG that is horrible to try to make lines with.  And, they are taking 50 lines per 1000 away because it is an easy platform.  Yes, I checked with the Department of Labor and they can get away with it.  Crap, I have to wait 5 seconds when ...
