Which verbatim is it? Your's, mine or their's? - Flummoxed

Just worked on an ESL report (with unfortunately for me very, very, very little English).  This is a VERBATIM account.  Sent it to QA and it was "transformed" into English with little regard for that pesky word VERBATIM.  Does verbatim mean different things to different people?

When I argued that a report was supposed to be "verbatim" they told me most accounts weren't "strict verbatim" and we were allowed to tweak them. How are we supposed to know which ones?!

I've been slammed for both correcting grammar on a verbatim account, and leaving the grammar incorrect, as is (which I hated doing). Now I correct the grammar. There have been ESL dictators on my account (which is strict verbatim) that have complained about their reports being changed, but know what? I say to them, You learn to speak English properly, man. I refuse to produce work that is subpar.

See, I feel the complete opposite as you. If we are told the account is verbatim and the dictator cannot speak English -- I feel "If you are smart enough to become a doctor than you are smart enough to learn English if you are working in America." If they sound like pure idiots it is on them. Mind you I feel completely different about ESLers who are really trying to speak properly -- I will bend over backwards to make them shine. What gets me more is the MME making changes they are not authorized/qualified? to make. But, I've been told it is strickly on them, not me........

the doctor is dead wrong about his anatomy? The patient has a problem with his foot and leg, but he dictates ulna. I wouldn't give it a second thought to leaving it blank, but I already got notice about documents with blanks (usually 1-2 blanks if any, and anymore than 3, to QA). The reason I ask is because this morning I'm doing an acute care progress note where the dictator says "The patient has end-stage chronic obstructive pulmonary disease and she is still sounding pretty crappy and congested."  I absolutely refuse to use the word "crappy" in a medical document if it is not within the context of a direct quote from the patient, i.e. "I feel pretty crappy..."  So I decided to omit crappy and just say "she is still sounding pretty congested."  QA w ...

Just got a call from my dr's medical records wanting to know the name of the dr she wants my records sent to (she and I looked it up on the computer during my visit and I saw her write the dr's name down) and my dr's address, which I'm sure was on her request, her name and address.  One of the last times I saw her, they put someone else's vital signs on my records (height, weight, BP and BMI).  Sheesh. ...

to fix it and put my schedule in there again.  Not sure how that could affect PTO, if you are working supposedly with no schedule.  But anyway, just a heads up.  Especially if when you go to turn time clock on, if there is no blue indicator on the today's activity plan field.  It that is empty and you are scheduled to be working, make sure the timecard is showing your scheduled hours. ...

What Does Verbatim Mean In VR Reports?

This is almost/sort of a poll. I need to know what verbatim means to you in VR. Do you change any wording? My understanding of verbatim is word-for-word (Merriam-Webster dictionary) unless a sentence does not have proper structure; if the doctor rambles on and puts the word 'and' between words where there should be a period, etc.  (Not included in this is changing mg to mcg in a wrong dosage, changing the medication itself if wrong, or other major medical erros&nbs ...

If not specified, is it better when taking an employment test to type verbatim or to follow transcription rules? Not asking for a specific test or company, I have just noticed that often times no instructions are given, and I don't know whether they are looking for following transcription/AHDI rules or just running the test next to a template in the system. ...

It got my shifts wrong for different days, putting 5 instead of 6 hours.  It evened out mostly to be correct but put an extra hour of PTO on my schedule. Which, my CCM, pointed out, I got paid for but my concern is if it keeps messing up on this stuff, how could I be being adversely affected.  Nothing here works as they want. ...

Is it me or what - I was always told to follow account spec and make sure I didn't use the forbidden JCAHO abbreviations. Lately if I look at an old report to get a slurred/muffled word that I can't make heads or tails out of, I've noticed that others on my same account don't follow the specs at all!
