Question on QA and changing things in reports - sm

I didn't get gigged on this..But I had a report go through QA where they changed the vital signs. I had a QA marker pretty much right after, so I guess it skipped back a bit into the vitals. What they did was change the vitals to the way they were dictated originally but didn't listen to the report all the way through. The doctor specifically mentioned to correct the vitals to what I had in there. So my question is, when QA corrects something, even though it was not marked to be corrected, does it show a correction was made to the original text? My friend transcribes for another company and anytime a correction is made, right or wrong, the person changing it is linked to the correction.

Definitely send this info to head QA/performance coach (or whatever they are called now). That way, if it gets audited, you definitely won't get dinged for it. Even if it doesn't get audited, that QA person needs to know they overstepped their boundaries and, God forbid, erred!
Feedback markers aren't automatic - sm

An SHDA can edit anything on the report without having to leave it under a feedback marker. Any error that is significant outside a blank warrants a 100% review. As much as to check for other errors as it is to make sure the dictator didn't correct himself/herself later. Definitely report this to your PC. They can see any changes that were made by the SHDA regardless of a marker.

Changing Transcription Jan 18, 2010I recently discovered that another employee has been making changes to transcription that carries my initials along side the doctor's initials, which indicates me as the transcriptionist.
I feel that this is not only inappropriate, but possibly also illegal.
