I had put that the patient had "intimate slurred speech" -- (that's what my ears heard!) -- but dictator had said "intermittent slurred speech".  SHEW!  Brain and ears must work together, always!!

She was stating the lab values, and when she says "chloride" it sounds like "fluoride," so that's what I typed! It wasn't until I got her again a few reports later and she said it again that the image of me typing "fluoride" sprang into my mind. I alerted my supervisor (embarrassing!) and she was able to fix it before it went out, fortunately.

Leaving blanks - Is becoming acceptable

I was talking with a colleague who works for one of the big nationals on a VA account. She said there is virtually no QA. If she leaves a blank, she never gets a note about it from anyone. She is very conscientious and does stop and take the time to research but she says if she didn't it wouldn't make any difference. Then I talked with someone at my local hospital that uses escription and that it is so bad, that it takes longer to edit than to type from scratch. So to stay within TAT, they rush through it and leave blanks and the doctors are all complaining. Wow, there is going to be a big lawsuit one of these if there already isn't one in the works.

The patient was having a colonoscopy and removal of hemorrhoids. I was so glad I found this before it went through the client. It was funny when I look back at it now, but I was stunned when I first caught it.

At the end of the procedure the doctor dictated his usual blurb, but I made a huge typo. "The patient will follow up in Dr. So-n-So's office in a year or on an a$$-needed basis."

For some reason, I don't use a shortcut for hepatitis, and I always type hepatits. Then there's consult, which I tend to type as conslut. Fortunately, spellcheck catches those, but I wasn't so lucky with "fluoride."

I did my last ASR for MModal last Friday and started my new job today. My heart goes out to all of you trying to keep your heads above water with Mmodal. Some KSMTs posted they got paid today, some got paid on the 1st. This tells me this was not an issue with late line counts or the payroll guru asking them to kindly roll payroll ahead a few days. Truth has to be they do not have the money available to make payroll and had to pick and choose who to pay based on the money available.
