This doc is making me nuts - With her phrasing

She says things like "He currently has a G-tube of which medications will be administered."
Or
"...secondary to UTI, Foley catheter associated, of which this has now been removed. "
Or
"this is a chronic problem of which he had a G-tube inserted in October"
She's making me nuts tonight. These examples are all from one report, too. I guess if that's the worst thing I have to deal with tonight, tonight is still pretty good, but I just want to send her back to English 101 and NO, she's not an ESL.

The combination of dictators who are totally ignorant of correct grammar and ASR that thinks commas are the only punctuation, we spend more time editing garbage than usable information. Oh yes! Let's not forget the dictators (most of them) who totally ignore HIPAA regulations and we have to waste even more time correcting that, which inhibits our ability to meet line quotas. I'm tired of everything being the MTs fault! Where is the culpability on the part of the medical professionals.

[ In Reply To ..]It's her voice and style of speaking, so I don't change it. That just wastes my time and messes up the VR. I have several who have idiotic ways of speaking like that. I actually think its more because they are young. Kids these days... :).
The only time ill really mess with sentence structure is with one of my really, really bad ESLs and their syntax is all off. I'll make that a little more Americanized.

This is the actual sentence the doc dictated. I listened to it 3 times. "There was initially significant tachycardia where her beta blocker was resumed, although this had given her bradyarrhythmias and sinus block in the past, but ultimately, with regulization, her potassium and magnesium levels, and because of recurrent hypotension her beta blockers were discontinued and her rhythm remained stable." HUH??????? ...

When I am transcribing, I am hitting some key combination that is turning my type to all caps. It's not the caps lock or it would show it was on. ESL, after saying the headings, repeats them along with adding a few more words, but it still means the same damn thing, says "umm you know" after almost every other word, keeps saying "feeded" instead of fed. A couple of reports ago she's reading a radiology report and says very clearly "primitive destruction" and then goes on to spell it "permeative."
