Profanity in a report... how do you handle it? - MoMT

I have a dictator who says:  The patient says "I look and feel like sh--."

I'm not sure how to handle it.

As long as it is quoted text, type as dictated - sm

I always put quoted text exactly as dictated.

However... if you really don't want to put it in there, you could put sh** or something along those lines, but I think I would ask a supervisor or lead before I edited quoted text.

If you are not stuck with verbatim, I would put what the patient said in quotes and put exactly what they said.

Anything in quotes is always transcribed exactly as dictated - including profanity

If a dictator specifically says something in quotes I personally think it's important to be typed verbatim ~ even if it includes profanity.

Depends on the client. For one of my accts, profanity even in a - quote has to be blanked and flagged. nm

We transcribe whatever is dictated, especially - sm

if in a quote. It lends itself towards how the patient is feeling, especially in psych reports. If not sure how to handle, check with your supervisor. Every company is different.

sometimes I will use a couple letters followed by "..." No one has ever complained that I didn't spell it out in full. It just makes it a bit less offensive to read (or transcribe).

I've been working 2 months. I have a psych doc who always says "the patient was less complaintive today."  I've changed it o "the patient complained less today."  But the doc sends the report back for correction and wants the word "complaintive" which is NOT a word!  I've been giving it to him, but man it drives me crazy. The Use Of A Or An In A Report.

Can anyone state the rule for proper usage of a or an?  I know you use "a" when the noun you are referring to begins with a consonant and use "an" when the noun you are referring to begins with a vowel.  However, is there a rule that applies to usage of a or an when it pertains to the below?  This is an 84-year-old female, who has a history of dementia - OR This is a 84-year-old female, who has a history of dementia.  Thanks, bfp ...
