never says "unquote," and you're left having to guess where the quotation ends?  Sometimes it's obvious, but other times, it could be anywhere.

at the end they say "quote, unquote." yeah??? which part??? duh!

They'll say, she said quote she feels like a band is around her head unquote. NOW, the patient did not say the word "she," she likely said "I." I remove the quotes around the pronoun 'she' and just put "feels like a..."

This is on the same lines as the quote thing.. they will intermittently say "comma" or "period" where they don't belong and then say nothing where they do belong.. Sometimes I'm just tempted to transcribe only the punctuation they dictate and leave out what they don't dictate.. just annoys me (along with the "quote" thing).

closing quotes at the end of the sentence. Maybe that will get him in the habit of saying unquote (she said sarcastically!) :)

run me nuts. One says period after almost every word. His sentences would look like this if I transcribed to his wishes: Patient is afebrile. Alert. Pain free. Stable.

Another doc will say parentheses and then never dictate where to put the ending parentheses. I have to just guess and his sentences are a mile long.

They are doctors, I guess, not English Language professors.

His sentences would look like this if I did what he wanted.

The patient is a 53-year-old female, she has been living a nursing home, last night she had chest pain starting at about 8:00 p.m., it radiated up her arm, it lasted about half an hour, it started again at about 11:00 a.m., it lasted for about an hour, her caregiver brought her to the ER, in the ER, .... that guy drives me up the wall.

I think this doc that says period actually means comma and that's what I use anyway, but the sentences would still look just as silly...just like your doc. Crazy, aren't they?? I usually just ignore their instructions.

habit. I have a doc who's notorious for that. He'll say #4 and then I have to back up and try to figure out what #1, 2 and 3 were. Arrrggghhhh!!!

If they start numbering way into the list, I just ignore them, especially in ASR. Way too much nonsense going on with ASR to worry about it.

Your days are numbered, Sweetie, especially if you work for an MTSO. If they're a small place and not planning on using the ASR, plan on them getting bought out by a big guy. Or should I say they'll sell out. It's still a free country and nobody is obligated to sell out to anyone.

I have this doc who will say quote unquote, so he's not indicating where he wants the quote to start, so then I have to back up to add the beginning quote at whatever spot I think looks appropriate. He uses quote unquote so often it's ridiculous. Most of his quotes unquotes seem unnecessary and make no sense at all. It's extremely annoying.

I type a lot of crap, but just had the worst doc I think I ever had.  Had 11 QA markers so I sent it to QA thinking they might actually LIKE getting some work since they are about to be history.  Anyways, this bi*ch marked 2 CRITICAL ERRORS and did an audit on it (which was still 96.5% in spite of everything I am glad to say).  QA you wonder why we have such a bad attitude?  That was totally unnecessary.  ...

I spend a lot of time typing for a huge university hospital clinic with no ADT.  Have to type in all patient information.  Almost every report is a LETTER.  Referring physician has to be typed into the ADT.  Then the letter recipient information has to be typed in and lo and behold have to re-type it into the cc list for the doc to get a copy of the letter.  I wonder why I can't run a decent line count?  I spend 50% of my time typing in information that I am no ...

(long pause) blah, blah um blah, um . . . .  This job is 24 minutes long packed with about 9,500 instances of using "um" no less than 4 times in every sentence. My line count is in the toilet, it has slowed my computer down. The spell check is missing things like doubled words. The headers are ridiculous.  AND I use the mouse a lot more because half the time the arrow keys don't seem to work. This SUCKS ...

Don't you just hate it when you've got a dictation that is so fraught with static, and it sounds like the dictator is 100  miles away from the phone, but you actually can make out the words, just barely, and you'd love to leave all blanks because it's such a chore to hear any of it, but you can't, of course, because you can make it out?  Several of these in a row is enough to make me want to punch my computer!  (or eat lots of chocolate!) ...
