medical insurance with decent coverage ($500-$1000 deductible, 10% copay for tests, hospital stay, ops, etc), single premium employee contribution no more than $250 monthly.

Transcend will not meet what you want. For single rate, $1000 deductible, - transcender

20% copay, is 340+ per month. Not sure what it is for family. It is very expensive.

I have Transcend, highest possible deductible - 60 per paycheck, 35 copay for in-network

and so far, I haven't seen an out-of-network doctor. They also paid for all my various ophthalmology testing (I have cataracts and impending glaucoma) without my having met the deductible.

I think it's set to go up to 70 bucks a paycheck, or thereabouts, on July 1. But still... not so bad. I'm single, no dependents.
