Would these things be two separate diagnoses?
Diabetic foot ulcer.
Charcot foot, left, with diabetic peripheral neuropathy.
Procedure was:
Excisional debridement, full-thickness skin and subcutaneous tissue, left midfoot diabetic foot ulcer. ...
This seems to be my week for needing help with words. I'm definitely hearing the doctor say "glibuteride" but I can't find any such medication. I'm thinking he's mispronouncing it, but can't figure out what it is. ...
Hello! :) The doctor dictated something that sounds like "astake" strips, and I'm completely stumped. The patient is a type 2 diabetic, and the sentence context is: "She will continue NPH 50 in the morning and then change her to NovoLog 8 in the morning, 8 in the evening. I refilled her (astake???) strips.
Please help!

Has anyone heard of diabetic liponecrosis? I'm not sure I'm hearing the doctor right. The patient has had painful nodules removed from her buttocks and now she has a recurrence of "diabetic liponecrosis."
Any help would be appreciated. Thank you in advance!! ...
"...Procardia which she bites to release the liquid, as she is diabetic s/l 'dash represis,' and I thought that maybe she is not absorbing her antibhypertensives." ...
In the diagnosis section of the consultation report, the doctor dictates:
"Uncontrolled hyperglycemia/HH and K."
My first thought would be hemoglobin and hematocrit and potassium, but he doesn't give me enough information in the rest of the report to be certain of this. The only values he gives are a blood sugar of 730 and a hemoglobin of 14.7.
Any thoughts on this? ...
Doctor dictates: "On initial evaluation, the patient was noted to have glucose of 560, with a bicarb of 11, and also has a (s/l something -demia) of 18%. The patient was subsequently admitted for further management."
I can't seem to find anything that would fit in the above space. It sounds like ?demia.
Does anybody have any clues for me? I would really appreciate it. Thanks alot. ...
Past Medical History: The patient was treated for a (cycle?) decubitus ulcer, stage 2-3.
It sounds like cycle, but google isn't showing anything for cycle decubitus ulcer. Please help! ...
I am on a report where the dictator keeps saying what sounds like sub-5. He is very clearly talking about an ulceration on the fifth toe. I've not heard this term before, maybe it is being shortened. How do I transcribe this? Please help if you can. I've googled this and can't verify it. Thank you. ...
I am taking a test for a company and have gotten as far as taking the transcription portion. I downloaded their file (pedalplugin-win) but whenever I lift my foot off play, the tape rewinds to beginning. I cannot rewind or FF and autoplay to 0 isn't registering.
Can anyone suggest what I could do to?
Thanks!!!! I have 4 hours to complete test. ...
Just had one dictate that this teenager took: Insulin 300-unit meal bolus plus 1 unit per 15 g carbs. Considering 300 units is a whole vial, that could be deadly.
Looking back at old records it was supposed to be: Insulin 300 units/3 mL. Meal bolus: 1 unit per 15 g carbs. ...
In a verbatim account a doctor is saying "...operation for exploratory laparotomy and perforated gastric "ulcerectomy".
Any suggestions? I cannot find an exact match for ulcerectomy... ...
It actually sounds more like "ulcer debrided in a s/l dermolating manner," but I have no idea. This doc often slurs, mixes words and makes up words, so I'm clueless here. This is for a wound care doc in an ortho clinic.
Does demyelinating manner make any sense?
Thanks!! ...
Operation of debridement for necrotic tissue. "The patient's s/l bocastera/bocasterile foot dressing was applied." I'm bad on brand names (if this is one). Any operative expert MTs know what this might be? Thanks. ...
