Just had one dictate that this teenager took:  Insulin 300-unit meal bolus plus 1 unit per 15 g carbs.  Considering 300 units is a whole vial, that could be deadly. 

Looking back at old records it was supposed to be:  Insulin 300 units/3 mL.  Meal bolus: 1 unit per 15 g carbs.

Some of the things I hear are terrifying. I once did a report about a medical error where a patient received a dose of 100 units of Lantus instead of the 10 units she was supposed to receive. They had to keep pumping her full of D50 for 24 hours to get through it.

If my son with type 1 ever has to be hospitalized, I will be right there double checking everything they try to give him.

Don't get me started on how many times they dictate Byetta using mg instead of mcg.

You would think that with diabetes as prevalent as it is, all physicians would be trained in at least the basics of insulin and medication dosing.

They know the dosages, guaranteed. The problem is that they probably use voice recognition or the work is outsourced and someone that way got the dosage incorrect because the dictator wasn't clear and they weren't educated on the standard dosages for the drug. I can't even tell you how many scary errors I've seen this way that even QA misses because they don't pay them enough to check on medication dosages unless a QA marker is left, which in turn, could make their bottom line suffer. I'm sure it is because of the decrease in pay they are all seeing. I hear QA only gets 0.3/line. Unreal! Everybody, even the best of the best, needs a second ear sometimes, especially whereas most of us in this profession do this for 8 hrs a day. It is really sad.

There is going to have to be something really bad happen that the news covers before somebody does something about all this medical transcription/EMR situation, isn't there? Unfortunately, this is what will be needed for people to understand; even then, they won't et it until they find that insurance won't pay for something because the documentation is wrong. I feel for the family this eventually happens to. It is unnecessary, but money seems to be ruling, so unless you say something to someone with some type of power, nothing will change. It is just so sad medical trancription has come to this.

Insurance had my gender recorded incorrectly - would not cover a surgery

I had a fairly common female surgery awhile back that certainly would be covered by insurance. When I got the bill, it said it wasn't covered. I called my insurance and found that I was listed as a male and therefore couldn't have such surgery. Okay, my name is very female, no hint of manliness at all and why would I claim such a surgery if I weren't female??? Here's the worst, it took 3 months of phone calls to get my gender back to what it should be. Around and around we go, called insurance: Not our problem can't fix it as you are recorded as such in your company records. Called company: Oops...we will fix that. Called insurance: You're still listed as male.

After getting sick of dealing with 'Peggy' on the phone and being transferred around only to come back to 'Peggy,' I finally asked for the boss, an American one, and you know what they did...changed it right then and there. What?!

Things are getting ridiculous.

Thank you so much for posting this! People have no idea what they are in for until they see someone that has had to deal with it. I guarantee you all, it won't be an overnight fix for sure and it will be more than a headache. I sure wish the patient's would look at this. That's a really unnerving story, and just ONE of the reasons I believe one day the wall will come tumbling down on the whole mess. How can a doctor "electronically" insert a physical into a patient's medical record. We were not created by McDonalds with a generic makeup. These are individual humanbeings! As for Peggy, any time I call somewhere and get her I don't even mess around, if there is the slightest hint I am not taling to an American, I request to be transferred to an American, and it works. I don't care if it is rude....

I Find It Funny Residents Don't Know Errors In A

I just did an ortho res who referred to a physiodesis with an A-plate.  This was clearly being said and I knew there wasn't such a thing, so I looked back at previously reports.  There were over 5 throughout the past few years with this A-plate.  Finally, I get to a report with an eight-Plate, which is what it should have been.  This just goes to show you how an error by an MT can be carried through multiple reports for years as many docs just read off them for their&nbs ...

A friend of mine was just fired from MQ with no warning whatsoever! Apparently she had CC'd the wrong doctor (very similar-sounding names, ESL dictator). I know this is a major HIPAA violation, but everyone makes mistakes...we are only human. My friend always had excellent QA audits above 98%, 15+ years as MT, no other issues with her work. doctor clearly states "a fair amount of adhesions;"  ASR transcribed "some adhesions."  That is so bizarre to me and if I wasn't really LISTENING and was just flying through proofreading, I would have missed it.  Not particularly treatment-changing for the patient, but certainly incorrect.  ASR just flat made up dictation there.  Absolutely amazing.  If I did that, I would be fired.  ...

In the diagnosis section of the consultation report, the doctor dictates: "Uncontrolled hyperglycemia/HH and K." My first thought would be hemoglobin and hematocrit and potassium, but he doesn't give me enough information in the rest of the report to be certain of this.  The only values he gives are a blood sugar of 730 and a hemoglobin of 14.7.  Any thoughts on this? ...

First, VR was going to replace us.  When that didn't work, now we -- American MTs -- are fine-tuning VR so the Indian MTs can replace us.  And to make sure this happens, MTSOs are paying us unfair, unlivable wages to force us out, thus forcing clients to accept what they get from the Indian MTs because there won't be enough qualified American MTs left.
