duodenal s/l sleep ulcer - Help please

This is dictated several times. I cannot find anything for "sleep ulcer" or "sweep ulcer." Any ideas please? Thank you!

Duodenal sweep: This is a term used by both radiologists and endoscopists, for the 1st and 2nd segments of the duodenum as it sweeps superiorly and posteriorly past the gastric antrum
Past Medical History: The patient was treated for a (cycle?) decubitus ulcer, stage 2-3.
It sounds like cycle, but google isn't showing anything for cycle decubitus ulcer. Please help! ...
I am on a report where the dictator keeps saying what sounds like sub-5. He is very clearly talking about an ulceration on the fifth toe. I've not heard this term before, maybe it is being shortened. Would these things be two separate diagnoses?

In a verbatim account a doctor is saying "...operation for exploratory laparotomy and perforated gastric "ulcerectomy".
Any suggestions? I cannot find an exact match for ulcerectomy... ...
It actually sounds more like "ulcer debrided in a s/l dermolating manner," but I have no idea. This doc often slurs, mixes words and makes up words, so I'm clueless here. This is for a wound care doc in an ortho clinic.
Does demyelinating manner make any sense?

The patient has sleep apnea and doc says probably have to come back for s/l refeeding and possible face mask and pressure use.
Could be repeat, but did not know if the term refeeding is relevant to sleep apnea ...
I just did that. I was just so tired that I just transcribed a whole 4 minute job while doing what I think was sleeping. If felt good so it must have been some kind of sleep. I was saved by the phone. My husband called and woke me up and I looked up at the screen and it was an utter mess! I repeated lines over and over and typed gibberish. On a sleep study/pulmonary somnography report, under the heading "Paramaters Studied" the doctor says "MM8." Does anyone know if this is correct or should it be MM-VIII or other ? I can't find anything on this with googling. Thanks. ...
Doing a consultation in which the doctor is trying to figure out if the patient's has sleep apnea. He says a list of reasons:
"...history of elevated body mass index, snoring, large Mexican friends and hypertension."
I'm sure he can't actually be saying large Mexican friends, but I can't hear anything else! ...
