I have never had that term while transcribing but several years ago when I ruptured a disk, my neurosurgeon stated that I was listing to the left when I walked. Hope this helps.
This patient is in for groin pain, diagnosis of hip early degenerative arthritis, DR states in HPI that she has a "history of s/l shark lordosis"
Under PE he states she has "no sensory deficits of lower extremity except for baseline s/l stockings of glove distribution secondary to her s/l shark-loida peripheral neuropathy."
I know that makes no sense but this DR sounds like he had a long night last night and may still have some substances running through his system. LOL He is usually ver ...
Pt with pattern one back pain status post MVA. The rest of the SOAP note is, frankly, exactly what you would expect. Prescribed Baclofen.
"Follow up if not improving or worsening. Did advise patient in regards to ____ s/l Zet-lie for resting the back five to ten minutes every one to two hours. "
FWIW it does not sound like side lay to me. It has a definite "Zet/set." She started dictating this about two weeks ago, has dictated three times. I've sent it in bla ...
She has 5/5 strength throughout the bilateral lower extremities with hip flexion, knee flexion and extension, ankle dorsiflexion and plantar flexion as well as extension of the hallux. Sensation is intact to light touch throughout except over the anterior aspect of her shin. She has a symmetrical 2+ Achilles and patellar reflexes. She has no evidence of clonus. She has a questionable s/l ** men-i-ski reflex. She has palpable DP and PT pulses. Thanks for any help!! :) ...
I just received my audit of five reports and in the diagnosis I put:
Acute and chronic low back pain.
I received a minor feedback and it was changed in the diagnosis to:
Acute on chronic low back pain.
Is this something new that I am not aware of? ...

So, what theories do we have about a system of slapping our wrists when 99% is not achieved ? Do we get fired immediately, or will they design some form of probation period to go it up to snuff ? Ho ho ho and a bottle of rum. Good thing the new "Pirates"movie is out this weekend. ...

I see there are 2 dislikes on the Smartmed job listing that was posted tonight. I sent in my resume a little bit after it was posted and before I saw that.
Anyone work for them currently? I couldn't find much in the archives about them. My only problem is that I have no recent acute care experience, I did work at a hospital from 1995 to 1997 and the rest of the time I have done various clinic jobs.
I just want to break into a company and get some acut ...

I'm going completely blank on this one. The patient entered the hospital for chest pain. The doc states: (blank sounds like gel, or jail)

In ASR I was told that when there is a list (docs usually say "next number"...) and the machine puts the # sign, that the "system would automatically make a numerical list, converting the # sign to an actual number".:
#....
#....
#...
One one report I had a report that had #, about three or four of them and I sent it through. I got negative feedback, because I had left the # in and didn't type out the number. Yet on others, nothing ever came back!
Which is correct?
