Which is it guys! Acute AND chronic back pain - or acute ON chronic back pain.

I just received my audit of five reports and in the diagnosis I put:

Acute and chronic low back pain.

I received a minor feedback and it was changed in the diagnosis to:

Acute on chronic low back pain.

Is this something new that I am not aware of?

I've done it both ways, but I do think 'on' is probaby more correct, but 'and' isn't necessarily wrong. I suppose you would just have to know if that is exactly what the doctor said.

It is acute on chronic back pain..meaning the patient has chronic back pain and now an acute episode on top of the already existing chronic back pain.

Acute ON chronic--especially if the patient is being treated for a current flare-up. In other words, he has chronic trouble (whatever it is) and is having an acute episode that needs immediate attention.

I do agree with post about it being "on". However, please listen to the dictator on this...sometimes they actually are saying "and". Got dinged for that before as well. It should be "acute on chronic", but heaven forbid you change it.

it basically means a severe pain that's on top of a normally persistent pain.

Pt with pattern one back pain status post MVA. The rest of the SOAP note is, frankly, exactly what you would expect. Prescribed Baclofen. "Follow up if not improving or worsening. She has 5/5 strength throughout the bilateral lower extremities with hip flexion, knee flexion and extension, ankle dorsiflexion and plantar flexion as well as extension of the hallux. Sensation is intact to light touch throughout except over the anterior aspect of her shin. She has a symmetrical 2+ Achilles and patellar reflexes. She has no evidence of clonus.
