Pt with pattern one back pain status post MVA. The rest of the SOAP note is, frankly, exactly what you would expect. Prescribed Baclofen.

Acute and chronic low back pain.
I received a minor feedback and it was changed in the diagnosis to:
Acute on chronic low back pain.
Is this something new that I am not aware of? ...
Any long-time MTs (over 30 years) having chronic neck/back pain, hand pain, wrist pain, migraines, etc., ever had luck at filing a workers comp claim? ...
OMG. I had no idea we could do this - I thought once we were "promoted" to MME we were stuck with it. I've never made so little money in my life than I have as an MME … it has been one of the biggest mistakes I've made in my life.
How did you do that, was it as simple as just saying you didn't want to do it anymore? Did your CCM give you a hard time about it? ...
Doctor repeatedly dictates at end of her office notes will see patient back peripherally. When asked about this, she says yes, that is what she means to say. Has anyone heard of "PERIPHERALLY" used in this manner in dictation? If so, what does this mean because I cannot verify this as being correct. Thanks :) ...
