ENT: Tympanic membranes intact with (positive?) light reflex.

S/L: Thyroid is without tenderness or mass. (Is this correct/makes sense?)

HEART: No gallops, peri-rubs with rales. (Is this correct/makes sense?)

CHEST: Symmetrical without mass or lymphadenopathy. (Is this correct/makes sense?)

What you are asking is a bit of a problem. It is not just a word or two here and there, but EVERY LINE. If you were trained, you would know that material. You would also know where to look it up.

I agree.....this is material you should know before you even enter this field. I am amazed at the untrained and obviously inexperienced MTs who are obtaining positions, while MTs who have been slaving for 25 years cannot obtain a position that is worth taking!! Please, do yourself a favor and study your transcription material until you are more familiar with the terminology. Patient's care and lives depend on the notes we produce, and from the above notation, it seems that you need either another course or some kind of mentoring.

You are hearing something very common, that MTs hear over and over and over.

I agree with the others. If you are a student, you need to study more and ask your teachers for help. If you are not a student, you have no business pretending to be an MT. Your questions here show that you are not even close to being minimally competent. Your attempt to impersonate an MT is hazardous to patient care and also gives competent MTs a bad name.

Someone also needs to point out that posting large portions of reports can violate patient privacy and confidentiality, so you need to be careful with that.

I think you are a student, though. You are posting too wide a variety of material.
