Several questions for x-ray report (Please read entire message) - happytiger2010

I have been working on this one for an hour and can't seem to get a few parts. One sounds like configuration. Also, after "FILMS OF THE CHEST", it s/l "One view" and then I think it might be a date afterwards. I am not sure. Please see entire message.

 

(s/l FILMS) OF THE CHEST: (s/l One view) (___). (s/l Compared): 10/12/10

HISTORY: Line tube placement.

FINDINGS: (S/l Configuration) is noted in the lungs. Endotracheal tube at the T4 level below the carina. A gastrostomy tube is noted and coursing in the direction of the stomach (??) which lies. The subclavian line is unchanged in position. Alveolar opacities are noted throughout the lungs, suggestive of pulmonary edema with bilateral pleural effusions. Heart size unchanged.

CONCLUSION:

1.    (S/l Configuration is ) noted in the lungs.

2.    Alveolar opacities noted throughout the lungs with pleural effusions and cardiac enlargement. Findings are suggestive of pulmonary edema. (S/l Endotrach) (s/l cannot be excluded) within the left upper lobe.

They can do several views of the chest, therefore "one view" makes sense. Also the compared is probably compared to the previous date.

"Configuration" could possibly be "consolidation." The G-tube would be going in direction of stomach (hopefully anyway) but not sure about "which lies."

Not sure about the "endotrach" -- is she intubated, if so maybe endotracheal tube and they do say "cannot be excluded" so it would mean that the tube may be in the lung but they don't know.

Don't know if that is any help, but are some possibilities.

Compared is most likely either compared to 10/12/10 or it could be Comparison: 10/12/10. (If a patient has prior films they always make a comparison to those films for any interval changes). If there are no prior films, alot of times they will say Comparison: None. or no prior films are available for comparison.

The endotrach would be Endotracheal tube cannot be excluded within the left upper lung.

Configuration would be Consolidation.

Chest one view. They will usually dictate how many views were obtained. Often times they will say PA or Lateral and other times they will just say one view, single view or they will say two views or PA and Lateral views. These are all standard phrases/terminology for a chest x-ray. Hope the info helps.

On your average day, 5000! Now, 70. I get that its a holiday, but I have run out for 3 weeks now. TSM said they over-hired and work went to India.. well, then get rid of the new hires already. This is ridiculous. Why would they hire more when they planned to ship more work overseas? Is this to train the new hires becuase they make so much less than some of us who have been here awhile? So far, I have had a lot of documents that require quite a bit of formatting - H&Ps, consults and ER reports from several facilities with each having different requirements and everyone wanting the ROS and PE different. Also, lack of punctation is hurting a lot too. I can't wait until I get out of here. Just 8 more months and counting! ...
