Hardware was removed. Does "holes in screws" make any rational sense in that context? You're on the right track, and the context should lead you to the right answer.

Also, is that the patient's real name? If it is, do you feel that it was appropriate to post it on the internet in association with a portion of her medical record?

I know of TFN, as in trochanteric fixation nail, but have not heard this before and am unsure what the other 'T' stands for, or if she just mis-dictated. I'm having trouble deciphering a few words in this sentence dealing with ankle surgery to remove intracuticular sutures.
