Patient is in for followup and ________. History is notable for the fact that she is doing better on Orencia subcutaneous 125 mg plus azathioprine 125 mg p.o. every day plus prednisone 10 mg p.o. every day and Voltaren.

Maybe the doc is saying, "RA, period," i.e., the patient is in for follow up of RA. The medications that you listed are for rheumatoid arthritis. Sometimes it is very helpful to use the clues around the word that you don't know to figure out what it could be.

Azathioprine has a direct effect on purine which in turn can cause gout.

This patient could possibly have gout, fibromyalgia, OA, RA, psoriasis...among other possibilities.  "Check rheumatology labs, including rheumatoid factor, anti-CCP, ANA, sed rate, CRP, CMP, CBC, Lyme.  The patient had a s/l 'dipsticks all the time.'  Check an ACE."  The doctor did pause after "had a" so that may not be part of the sentence.  In that case, it sounds kind of like, "The patient gets sticks all the time."  I'm not sure if either makes sense.  She does mention ...

Doc is referring patient for labs to rule out rheumatoid arthritis.  He's listed these a million times and always says "CCP level."  This time he said "citrullinated cross-linked peptides."  I'm thinking he just doesn't know what CCP stands for, as I can only find "cyclic citrullinated peptide".  He's not a rheumatologist, but then again, neither am I.  Can someone with rheumatology experience weigh in? ...

If the client profile makes no mention of patient name (yes or no) being left in the report, account does not specify verbatim, is the ADHI/JCAHO directive to type the patient's name or replace with "the patient?"  Just checkin'.  Am paranoid about everything with the audits these days.  ...

I swear to all that is holy, this is what this less-than-stellar PA just dictated:   "The patient IS a high-risk gestational trophoblastic disease..."  I went back and listened thinking I must have heard wrong and she said patient HAS ... Nope.  She said IS.  This is the same PA who dictated a creatinine of 92 for someone.   That sound you hear is me hitting my head on the desk. ...

Today out of the blue, got a call that I had done great on testing and they would like to offer me a position.  I said thanks but no thanks since I'm no longer looking for a job.
