CPAP cm of pressure - Cbyrd

When doing CPAP titrations, Dr states "CPAP was initiated at 4 cm of pressure".  I have seen different ways to transcribe this, either as written or 4 cm/H2O.  Which is correct?

When I get these reports, I transcribe what the dictator says.

If s/he says: CPAP was initiated 4 cm of pressure, then that is what I type.

If s/he says: CPAP was initiated at 4 cm of water pressure, then I type 4 cm/H20 pressure.

I just type as dictated, even if dictator says the word water. Gives me less to think about, less to edit on ASR. Unless there's something specific in one of your specs, I would type as dictated. I don't change the dictated word oxygen to O2, or carbon dioxide to CO2, etc., or vice-versa, either.

when it comes to combinations, I do. For example, I used L/min or mmHg (when millimeters of mercury is dictated). Things that make sense and convey information quickly and efficiently is what drives my decisions, especially at the recommendations of BOS.

I have seen a lot of MTs change the dictation of 02 sat to oxygen saturation just to pad their lines (not that I am suggesting that is what you are doing, but it is something that happens more frequently than most people admit to).

If I am going to change dictated water pressure one to the H2O, then I should change carbon dioxide, etc, to CO2, etc. So, I'm just not clear on why just change the water pressure one.

I type O2 as dictated. If it begins a sentence, I might change it to oxygen saturation. By I never change the dictated oxygen saturation to O2 saturation

At the outset, let me make clear my employer does not require verbatim transcription but I know that's a consideration for some people. Please utilize the following as appropriate to your situation. I do vary the output depending on what I know of the dictator.

CPAP and BiPAP, like PEEP, are measured in centimeters of water. Therefore, like BP or PEEP values, if the dictator states one part of the unit of measurement (i.e., "mm"), I include the other.

Ergo, if I hear "CPAP at 4", and the dictator is persnickety, that's what I transcribe. If I hear "CPAP at 4 cm", I transcribe "4 cmH2O". If I know the dictator to be casual in their style of dictation but appreciative of a professional looking document, I include the "cmH2O" even if only the numeric value is stated.

Please note, just like with BP and mmHg, there's no slash between the two - it's centimeters of water, not centimeters divided by water.

Additionally, as ICManiac said, I don't spell out those terms - they're accepted and understood, and spelling them out makes it more difficult for clinicians to quickly find needed info.

This doctor seems to use the terms CPAP and BiPAP almost interchangeably for the same patients.  I assumed this was just a mistake on his part, but now he mentions them both in one sentence.  Can both CPAP and BiPAP be used at the same time?  Thanks.  ...

I have a patient with sleep apnea and the doctor states the pateint will need a "break" CPAP at home.  The only other thing it sounds like is a "brick" CPAP.  I don't think it's either, but definitely not brick.  I've done an extensive Google search so danged many times today with all of the crappy docs I've had, and I am so sick of Google, I could just about rip my computer off my desk and smash it against the wall.  Either that, ...

Ok, I've been getting a lot of doctors saying this. That patients can go to/return to what s/l CPAP (see pap).   Is this correct? I'm not sure if it's a nickname for another local facility either. I have a list of commonly referred to places for this client and nothing like CPAP is on there.  Just curious. ...
