Physician office equipment upgrade... advice please - J

Posted: Jan 28, 2011

I just picked up a private account which is still using microcassettes and the quality is awful.  They are agreeable to update to digital equipment but I have no experience with any specifics on it since I do all my other accounts online.  I was hoping I could get some advice on what equipment to look (brands, etc..).  Would like to find something where he can save the file onto a SD card.  Finally, input on the "software" that is used with any of these systems.

Thanks,

I pretty much forced my local account to go digital when my last remaining tape machine died, and I didn't want to spend the money to replace it.

The doctor has a little Olympus digital recorder that works pretty much like his tape machine did, as far as the buttons he has to push. The recorder plus right into a USB port so the digital audio files can be transferred to any other device. I think his machine cost somewhere in the vicinity of 50 bucks.

Since I have always done pickup and delivery for that account (the office is only a mile or so away, and I enjoy interacting with the doctor and the staff), I just go in there and plug his machine into the computer, copy over the files to a folder on the desktop and then erase them from the dictating machine. Then I plug the thumb drive that I carry with me into the machine and copy over the files from the desktop folder to the thumb drive. I always leave the files in the folder on his desktop until I have transcribed them, which means the first thing I do is actually erase the old files from the folder. (For the sake of redundancy, they are stored on my home hard drive after they are transcribed, so there's always a copy somewhere in case we need to go back.)

All you need at your end is a foot pedal that will play audio files. I have a WAV player, which came with its own software, but there is a free program out there somewhere that will also play audio files using a foot pedal. It might be called Express Scribe, but don't quote me on that; I could be wrong.

If you currently have a USB foot pedal that you're using for your online accounts, it will probably work for the local account, unless it's some kind of proprietary system. If you have to buy a pedal, I highly recommend the WAV pedal. It costs a bit, but it does the trick.

So, best case scenario, all you will need is to download the free audio player, use your current foot pedal, and have the doctor buy a digital recorder--but make sure the recorder he (she?) buys connects to a USB port, so you can transfer the audio files.

Worst case scenario, you might have to buy a foot pedal--but if you're an independent contractor and have a business license to be that, you can write off all your equipment.

Pretty much any brand of digital recorder should do the job. One of my previous docs had a Sony, I think it was.

At times when I am out of town, or can't get out of the house for whatever reason to go pick up the dictation, the office manager has been taught how to "drain" the machine, and upload the audio files to a secure file site, which I then go into and download. I upload the finished documents to the same site, and she downloads and prints them. If you don't feel like making the commute to your account's office, you could probably work out something like that too.

It's really pretty easy, once you get it all going. Best of luck, and congratulations on nailing down a local account!

Need To Convert From Office 2013 To Office 2010
Apr 26, 2014

I'm starting with a new company next week.  My Windows 7 PC has Office 2013 Professional.  The new company's platform is only compatible with 2010 or earlier.  I don't have the money to spend on 2010, so I'll either have to decline the offer or try to find a way to install 2010 cheaply.  I thought I might be able to download a version for free, since it's not the newest version, but it doesn't look like I can do that.  Any ideas? ...

So, I'm editing this report and it comes to the end and the physician states, "Transcriptionist, thank you so very, very much.  I appreciate you and your work."  Totally made my day!  Since this physican doesn't know it was me doing the report, I wanted to pass it on so hopefully it'll make someone else's day too.  Please no negative comments.  I have enough negative (as we all do).  Let me just enjoy the thank-you, and even more the appreciatio ...

Resident Physician
Nov 26, 2010

I have alway been told when in doubt, leave a blank. Resident did not give attending physician's name. There was a doctor's name in the hearder as attending, but I  was not sure so I left the attending line blank at the bottom of the report. My boss calls me and tell s me to go on a limb and guess in the future.  I do not like those ethics because it leaves me up for problems in the future.  Comments welcomed. ...

Associated Or Associative Physician

The doctor I transcribe for sometimes says "Associated Physician" and sometimes says "Associative Physician" and then lists the doctor's name.   Sometimes he will also list a "Primary Physician."  Just curious to see if there is a difference between associated vs associative and if one would be better than the other in this situation.  Any thoughts? Nov 23, 2012

Below is a link to a NY Times editorial.  "The advent of electronic medical records has been a boon to patient safety and physician efficiency in many ways. PSA: Consulting Physician
Oct 03, 2013

I admit, this is a huge pet peeve of mine.  The consulting physician is the one who goes to see the patient at the request of the REFERRING physician.  I wish I could get some docs to read this and state the difference.  If I was typing, I could just correct it right away but I have to correct it on the VR.  ...

Just A Message From A Physician

Hello everyone and sorry to intrude. I just wanted to say thank you. I'm a hospitalist physician and I dictate daily. I would say about 10% of my dictations contain a "_______" that needs to be corrected, maybe less...and I am someone who mutters a LOT. So I'm surprised it's not a much higher percentage. And when I hear some of my fellow internationally-born docs dictate, I'm amazed that ANYONE, let alone someone over the phone, would be able to understand what this person i ...

Physician Do Not Dictate List
Mar 15, 2010

Since we have a (lengthy) Do Not Use list, I think the doctors should be versed in proper dictating form with a Do Not Dictate list to include "next", "next number", "new line", "skip line", "and", and "uh, uh, uh" and all those other little irritating words that ASR loves!  Charge them 3 cents MORE per line of dictation if they can't maintain 98% with no blanks sent back to them!!!!!!!! ...

Just got an E-mail from our QA person stating that a physician is complaining that we are not sending CC's to the PCP that he gives at the beginning of the report.  Now, I do realize that this information has been placed into the Client Profile under specific instructions for this physician, but sometimes when you are working on the same facility and feel comfortable you stop looking at the CP for every single MD.  This is not a facility I type on often, and apparently it is not j ...

I DESPISE The New Physician Lookup Feature.
Sep 05, 2010

So let me get this straight: I want to put a physician's name in the report.  I hit CTRL + N and start typing the last name. Before, it was nice.  The search would happen automatically. NOW I have to tell it to search (even for mandatory ADT names when you hit enter to get to the physician lookup)?  Why?  That is another keystroke that is wasted, and add to that, it was nice before that if you hit the letter 's' it would scroll to all the docs with last name ...

Question About Working For Local Physician - (sm)
May 31, 2011

I just have a question for anyone who works for a local physician.  I work FT for an MTSO.  A local doctor asked me last year if I would transcribe for him while his MT was on vacation.  I said yes and I go there next week to train.  My problem is, I don't know what to charge him.  His assistant mentioned it to me on the phone and i said I would tell her when I come to the office next week.  Does anyone who transcribes for a local doc get paid by the hour?  ...
