Don't you love starting your day with a few informative emails with the mistakes you made? It brings such joy to my life. But seriously, I will be giving myself away with this one but COME ON, I got a BMI wrong yesterday because evidently he didn't mean to give me a blah.blah-blah-blah%, he was "stumbling" over his words. The "helpful" note said that I should have consulted a BMI calculator to find the correct BMI. Sigh.... just another clerical duty, part of my job description...?

being foolish enough to decide to put something in and whatever you put would have been wrong, you'd have been off in your calculations, but protest that you used a calculator or consulted a BMI chart on line, you'd be told your calculations were wrong and you shouldn't trust everything you find on the internet.

Those people are out of their flippin minds. You have to wonder, what in the ever lovin H they are thinking. It's pathetic.

An MT is not qualified to QA a physician - OldMT sm

I don't work for Nuance, but I worked for an MTSO who tried this very same thing. My answer was that I am a medical transcriptionist and it is not my job, nor am I qualified, to do QA on a physician. That's what they have peer review for.

I transcribe what is dictated. Period.

If the physician dictates a BMI, I type what is dictated. I do not stop to use my own BMI calculator to ensure that he/she is correct.

If the physician dictates, "This is a 72-year-old...", I do not go back to calculate the patient's date of birth to make sure he/she is correct.

If the physician dictates in the diagnosis that the patient had an "I&D," I do not go back to previous reports to see if I can find out if it was an incision and drainage or an irrigation and drainage.

I will not go back to previous reports to make sure that the physician dictated the correct medication list.

All these things (and much more) are simply not my job. My job is to transcribe what is dictated. I will flag potential errors (i.e, dictates that aspirin is an allergy, but then lists aspirin in the patient's medication list), but that's as far as it goes.

Unfortunately I was told that it was my responsibility to make sure what the doctor said was correct, as in the labs are not out of the normal parameters, the dosage on meds were not out of line, etc. So basically what they want is for you to research all labs and meds, etc. I agree with you completely. If I am supposed to know all of this then I would have gone to medical school and become a doctor. It is up to the doctor to dictate the correct information. He/She is looking at the meds and labs when they dictate. It is crazy what is expected out of people that they do not want to pay their worth. I also was told not to spend too much time trying to figure out something - so what it is? Research all these meds/labs, old reports, or not? It is crazy.

I may or may not catch the aspirin discrepancy, then again I might not. I may flag it (on MY time, MY time), then again I might not. With certainty, though, I will not be held responsible for failure to do so.

As MT, for sure no deed goes unpunished. Darned if you do, darned if you don't. Chances are we would get dinged for whatever we did in this situation.

If a medication dosage or lab value is extremely unusual, I will flag those. As an MT, I think it is my job to know normal lab ranges and usual medication dosages. And I think it is my job to flag anything that seems like it may be in error.

But many of the things that my former MTSO was asking for were NOT the job of an MT. Medication dosages and lab values change over time, so it does no good to look those up on an old report.

It is also not my job to verify that the physician is dictating the patient's BMI correctly. In order to calculate the BMI myself, I need the patient's height and weight, and how could I be sure that the physician dictated those correctly? It does no good to look that up on a previous report since a patient will often either lose or gain weight. So, I'll either type it as dictated or flag it.

Let them PAY for my time for flagging, let them put me through a course in Pharmacology or Lab Technology, maybe then I'll flag/QA. MDs & nurses are making the bucks, not us. Medical Transcription means exactly what it says- NOT Medical Second Guessing (MSG)or Medical Flagging (MF).

Too much, too many sacrifices are already being required of us. Next, MTs will be sued for NOT catching the dictator's error...medical knowledge which we are not required to know.

In the past, whenever I have pointed out that all this crap, verifying, flagging discrepancies, demographic research is uncompensated, I was told by the MTSO.. "It's part of the job". No, it's not part of my job.

You cannot be legally be consistently required to do anything not paid for as "part of the job".

I'm sure they may find a loophole or a way to get around this; think of the $$$s they'd lose, if they actually had to PAY us for this "required part of your job".

One issue, however, should be nonnegotiable: Dinging/docking your pay for perceived errors or omissions for UNPAID WORK.

As an hourly employee, O.K... but as a Per Line employee? I don't see how they have been getting by with this for so long.

We all need to keep a daily log with all the unpaid work we do, both on a minute by minute as well as how many characters/lines we do UNPAID ... even if only a conservative estimate and SAVE THAT LOG. They are forcing us to do all this unpaid work under the threat of termination, so we won't actually be able to use this log until we are no longer with the company, but it's worth a try.

So now we have to second guess - NOT MY ****ING JOB

the mumbling, moron MDs? We're not presumed to have medical knowledge, only to understand and spell it. That's why they get the big bucks while we get pennies. Same goes for lab values, common/standard doses of medications...I type what I hear, end of story. It may sound harsh, but so does losing my home, car, food stamps or my family doing without.

...you may get hit with a major error. If you leave a blank, you should have looked it up on google or previous report (per a few QA--not all) so points off for "invalid blank," and if you blank it and QA cannot get it, then the MT may get points taken off for an "invalid blank." Seeing a pattern here? Blame the MT regardless. Don't rock the boat asking the dictators to enunciate and double-check their data.

Do yourself a favor and SM - Save all those e-mails

Having saved them may come in mighty handy when trying to dock your pay for not using knowledge required only of MDS.

Also, if the issue of termination due to quality issues ever came up in the event of an Unemployment hearing, you will pat yourself on the back after that first UE check for your family comes in.

I would have gotten a reversal & SENT HER/HIS STUPID E-MAIL TO MGMT'S LEGAL DEPARTMENT. Required to use a calculator now? What next - use your imagination!

Save that e-mail, if for no other reason than to establish a pattern of stupidity.

I knew I shouldn't have looked at my FIESA from Saturday but I did.  Majority are 100% so for that I am grateful but THEN there are the others where you are marked off for stuff that make no sense and then there are the stupid errors that I did and will own up to and wonder where my brain was when I was typing - I think my brain was so worried about being marked off for every little thing that I honestly can't concentrate sometimes like I need to. ...

I know we complain and complain about this on here and we fight and argue with the TLs about this...I was marked off half a point for EACH blank that I left on a dictation that the tape cut out.  The only explanation from QA was that "it was clearly understood".  I am just wondering how it was clearly understood.  I went back and listened to the tape and sure enough where I had blanks, the tape was cutting out.  So why am I being marked off for blanks.  Then on a few ot ...

Okay, first quarter FIESA reviewed about 80 reports of mine and I actually got my incentive.  Now for this quarter, I am wavering on not getting my incentive with a 98.88 and for the past few weeks they have not reviewed a single one of my reports.  I smell something fishy.  This whole FIESA thing is to a) review enough reports a quarter so you fall below 99 and then b) if you are below 99, stop reviewing reports so you are stuck there with no chance to bring it up!  How fric ...

our post-audit reports are randomly pulled.  If you score high enough to get the bonus, your reports are gone through by the hatchet queens in India until they can find a report they can "audit" to bring you below 99%.  If you are kicked back to 98.99%, they still meet their contract requirements to the client, and Nuance does not have to pay you a bonus.  ...

I almost always get my critical errors on one particular doctor who I think I understand and then it turns out I miss something.  I think there is a way to do a report on critical errors and see if I can trend the dictator but I can't figure out how.  Does anybody know?  And before you tell me to ask my TSM I did but as usual never got an answer. ...

Much as I dislike FIESA and all this QA, what I really hate is finding out I've been typing something WRONG for 30 years.  The only thing I can say for myself is some QA decades ago filled it in for me and I didn't look it up.  Been typing Campo's fascia instead of Camper's fascia. Took a 1-point post-audit hit on it, too.  Shakes me up, what else have I been doing wrong for 30 years?  And will it knock me out of bonus territory next quarter... ...

I must admit, I never looked at FIESA, not only because it was depressing and humiliating, but its not the most user friendly site. My manager was constantly badgering me for not looking at it though and actually had me try to get on the other day when she called. How do other people get away with not looking at it? Just for "fun," I counted how many times I prevented a critical patient safety error today by catching the speaker saying the wrong age, gender, right vs. left, hyper versus hypo, you get the picture.  I quit counting at 8.  Never in my many years at Nuance have I made 8 CPSEs in 1 day.  So are we getting credit for the good work we do?  Not very likely. ...

Got a report back below 99%--Actually 98.7% with only 0.25 point reduction...for what?  Not typing DICTATION ENDS HERE on a report that only had part of the first sentence dictated....and I have to sign off that I reviewed this?  The report was sent to NHR for review with a note--INCOMPLETE DICTATION.  Okay, next time I type dictation ends here, but scoring this is ridiculous.  Why is this even an error--should have just been an  FYI--the report was never going to make i ...

I was having a good day, until I went into Fiesa...I have decided that one particular dictator--who speaks what he probably considers fluent English (He is American), is going to review for every report because he is the one I get poor scores on... I am constantly guessing if he said "and" or "in" or "on," and I missed a period....he backtracks, leaves words out, stumbles, and CANNOT pronounce even the simpelist of medications...let alone spell them, he spells them wrong too, hard to even under ...

I am always above 99%, but just below 99.5%. It is destroying my confidence and my ability to concentrate.
