FIESA MAJOR ERROR - PasMT

So I check FIESA this morning and they give me a MAJOR error for typing "and" instead of "on" on a very poor quality recording... As in..."we will need to see on whether we need to check" and I put ....." we will need to see and whether we need to check"
I told my acct manager - I said really that warrants a MAJOR error - she replies
Yes, really............ I have THE WORST account manager in the company - she NEVER EVER EVER sticks up for me.. maybe she hates me - feeling is becoming mutual!

[ In Reply To ..]And you cannot convince the QA person even when you show her in the dictionary, she still won't budge because she has never heard of it, so therefore the doctor could not have said it. I have become convinced that all this QA actually degrades the original work product.

I got a major error because I missed a blood pressure error (I hate that because it's not like I "typed" the error, I missed the error the stupid machine got wrong....) anyway, so now if I am not 110% sure I will pend it. So now, I get dinged and the note says "I was able to get the proper reading when I slowed down the dictation...." SERIOUSLY? You know how they say CURRENT MEDICATIONS Include.... I got a major error for leaving off INCLUDE: .... and a huge paragraph explaining why that is necessary. I am constantly looking up old reports to look up doctors, meds etc and find tons of mistakes on old reports!

[ In Reply To ..]Don't feel like they are picking on you because I recently got a 1 point deduction for a statement that I have in autocorrect (because they say it all the time). I know for a fact that it was changed because I checked in autocorrect to make sure I was right. However, my TSM is as useless as , well you know. I protested, copied, sent reversal request, and I hear NOTHING. She just wants to get her check and never mind about the people actually working.
KARMA is a witch, you know. And it will catch up with her. Next time, I'm going over her head with a cc TO HER.

Also, there *is* a difference between CURRENT MEDICATIONS and CURRENT MEDICATIONS INCLUDE, and it should have been transcribed (or edited) as dictated.

[ In Reply To ..]My point is, they assign us a percentage but WHERE is the percentage of errors the report starts with? There are some reports where there is literally something wrong every single line, and sometimes of course more than 1 mistake per line. Fact is, I have been doing transcription for 20 years, editing for 2. I think transcription is much easier and I'm sure my accuracy was right on as a transcriptionist. It is a lot more difficult in my opinion to fix someone else's garbage. It sounds like you're a TM....

[ In Reply To ..]What this and other companies have done to us is painful. I have taken a 600 dollar a month pay cut when VR started. I have TWO former coworkers who lost their homes. Companies like this take advantage of us, make us work for peanuts, make us jump through hoops, and humiliate us. The only reason I'm still here is because I have an autistic daughter who can't get on the bus by herself but soon I too will have to go out and find a job that actually pays at least minimum wage and my autistic daughter will have to come home to an empty house, at least for a few hours. There are some 15-20 minute reports I come across that are loaded with errors and take forever to fix everything and even then, there is a ton of work to go through word for word. I know I'm not even making minimum wage when I get those...

[ In Reply To ..]My 2 cents but an error in someone's BP is a critical error (yes, I am QA but not for Nuance). To me, that ranks up there with a lab value error or a medication dosage error. Interchanging words such as "on" and "and," while not significant in this situation, could be as they change the meaning of the sentence. I find VR tricky; your mind can be fooled into thinking that what you see on the page is what you hear. I get finished reports back from my client that have already actually gone through proofreading and QA on the client's end that are still pretty messed up. In many cases, I could probably have transcribed the report myself. We're not doing this to be mean to you or rob you of your pay but we all know accuracy matters. I try to treat every document I QA as if it were my chart; I know how I hate seeing errors in my own reports. I'm sorry the pay stinks, but going through a ton of work word for word is what you were hired to do. Try doing QA on someone else's work some time to see how the other half lives. Our jobs are no bed of roses either.

An erroneous blood pressure is the LEAST of concerns on those reports. WRONG MEDICATIONS, WRONG DOSAGES,the doctors do not proofread for the most part. Many think it is beneath them. Those reports go through. Just face the fact that NUANCE is just one step above sweath shop label. And you should be ashamed of yourselves, every one of you, for treating hard-working MTs in this manner. I saw an e-mail to a coworker of mine more or less stating that a MONKEY could do this job...in so many words. Your company is the WORST thing that has even happened to transcription and you are a blight on the entire industry.

[ In Reply To ..]Just an FYI regarding VR . . when I first started working in VR, I hated it and a former boss of mine told me one day, she said "if you would rather type the report and feel you can do it quicker, then go ahead and type it." So, some of us would delete the recognized report and type it from scratch. Not sure if you want to try that or not, not sure if it's okay with this company. Just letting you know that's what several of us were told at another employer.

[ In Reply To ..]Don't feel bad . . .I'm a QC at Nuance and I got a major error for leaving off "include" under the medication heading too. It is ridiculous, I agree. We're all in the same boat here, whether we are QC or MLS . . . we all get marked for the same things, so don't feel alone.

Thought I was going to make it through today with no Major error but no such luck. I typed "There are" and they heard "The" Trust me either way the sentence says the same thing and in no way changes the meaning of the sentence so I get dinged a major error for this. I really hope these quality people get good bonuses for these type things. Just another major error to add to my very long list. More in the last 2 weeks then in my entire career of being a transcription ...

.....for missing a pronoun at the begininng of a sentence... Symptoms started instead of His symptoms started....major error? I think not. Minus 1.0 whole point for me! There is no consistency in the grading of reports. And again I say, this is a subjective error...someone heard it, I did not and it does not matter anyway. This just tops of my bad day. I get on to check previous reports from yesterday and see 1 correction on 1 report out of all of them. QA filled in my blank, and the error category is "major" with subcategory "not applicable." Platform is Cornerstone. Since when is it a major error to leave a blank for something we aren't sure about? Keep track of every job you send to QA with blanks or other. Compare job numbers to those jobs that come back with major errors. Want to bet that most of those jobs we have a hard time with are the very same jobs that we are getting marked down for? Most likely the very blank or the very problem we had is what the major error is for. ...

This is a new one to me and laughable honestly. I thought we were to spell phoenetically a doctor's name in the text if we cannot find it or verify it. So this is what I did as usual and got a MAJOR CRITICAL ERROR for putting the wrong doc name??? WTH. Seems like they are grasping at straws now. What's the deal? We are to be mind readers now and get these names exact every time even when they don't spell, we can't find, etc. etc. Nuance just makes itself look ridiculous ...

I used to almost NEVER get error notifications in Fiesa. Now that the new "compensation plan" has been put into place, I get a notice every other day that they found an error. Due to my work ethic established over decades, I have not sped up my QAing of reports at all, accepting my fate that I will now be making third world wages until I find a new employer. Supposedly post-audit has managed to find these errors in my work. I don't buy it for a minute. How much lower can Nuance go ...

After starting with Nuance, getting great QA reviews, great post audit reviews, and thinking I was doing well - I have gotten THREE major alert emails in 2 days. I think I am going to do what a poster below suggested and block them. I don't see that having the reviews come through helps me in any way. I have also noticed that some of these jobs have been QCd twice for some reason - and it is not a post audit.
