When do you send to QA - CT

If you have 10 reports all with blanks, when do you determine a report is routed to QA versus striaght to the client? How many out of 10 would you say you send?

Its my understanding that if you have over 2 blanks in a report it should go to QA. The client profile might indicate something different, however. I believe if you have multiple blanks on a job with extremely poor audio, you can simply add a QA/QC note and send an e-mail to your QA lead -- just my understanding.

The account I work on has really hard doctors - even QA has a hard time filling in the blanks - just depends on your own situation.

Probably 40% of reports with blanks! - (sm)

I send to QA if:

a) I have tons of blanks

b) if I think it is a term somebody else might be familiar with, second pair of ears

c) filling the blank might make any difference in patient care, so even if I think QA has a fat chance of filling it in, I figure it is worth a shot

d) I am really hesitant to send a report to QA when it is a really long report, unless there are just a boatload of blanks in it.

Having said that, I have about 10% of my reports go to QA so I am penalized every pay period! My % to QA was on a downward trend, last down to about 8% until my group assignment was changed recently!

Oh well, I figure it is a chance to learn and be more productive next time when I don't have to hem and haw over the same thing.

IF we felt the blanks could filled in by someone else. I also send if it is something I'm not comfortable with and think QA can help. Otherwise, I do NOT.

crappy sound quality that no one would be able to figure it out so no need to send to QA when it's that bad.

1 blank for a couple of my accounts, 2 blanks for the other accounts...SO I have no choice. I know I would never send them if there was a chance of penalty, though.

on-call back-up MTs for the Labor Day weekend? Makes me laugh. She always expects this unforeseen deluge of reports because, gosh, you just never can tell when those doctors will start dictating hundreds of reports over the holiday. one sentence. YOU HAVE TO REDICTATE THIS REPORT BECAUSE THE AUDIO IS SUCH CRAP THAT I COULD ONLY HEAR EVERY OTHER WORD AT BEST. Seriously! Okay, it was the end of the week and on my very last document I had 1 word that I could not get - so I sent it to QA. Lo and behold! The entire document was picked apart for ridiculous things and some things were even changed incorrectly. All that and my blank space WAS NOT even filled in! No more QA for me! ...

...no matter how many blanks, isn't that what QC people do? I mean, at some point it goes with blanks if no one solves the mysteries, right? Why would an MT be required to do this also?
