I got a call from MQ regarding the amount of <skip> markers in my ASR reports.  I was told that there shouldn't be more than 3 to 5 skip markers in any given report???  This sounded absolutely ABSURD to me and I told them to give a listen to some of the reports I edit and explain to me how exactly I am suppose to accomplish that! 

Has anyone been told that or something similar?  It doesn't seem to me that the "ASR machine" will learn if we simply do corrections without insertion skip markers... 

Can someone enlighten me?  THANKS A LOT!

Enlighten you? No. There is no way to shed any light on this issue. This is how MQ is. This is how ASR is with them. They want us to edit reports perfectly w/ imperfect tools and make as many lines, more actually, than on straight transcribing. I do not see this changing ever with this company. They want 10% QA submissions, 150 lph minimum, which should get you about minimum wage, at 98% quality, and they want us to be happy doing it.

I for one am opting out. Not a healthy way to spend 40 hours a week, and I have grown accustomed to having a roof over my head and food in my stomach. I do 100% ASR so I am screwed as screwed can be, and I cannot take this anymore.

The ASR machine does very little learning, don't see that changing either.

What a dumb thing to tell you. That's impossible! Why don't they just tell us to make those horrible reports pretty just for funsies and less pay!? I put in as many skip markers as a report needs PERIOD.

I've never been told anything about it. I have never noticed that the ASR machine is "learning". my reports on my usual dictators are the same mistakes over and over and over and over, no matter now many skip or redundant markers I use. I have even sent requests to review the qualifications of the dictator to "support" but NOTHING CHANGES!

I was told just last week "thanks" for being willing to report repetitive mistakes by ASR but that most of these cannot be changed because if the word ASR is putting in is a real word, it will not change. So input from us is going to make little difference in my opinion. NOTHING IS GOING TO CHANGE. I assure you. I have over and over again, for months, put in redundant markers and ASR never seems to pick this up. I think its hopeless.

They want to pay us minimum wage for our knowledge, our carpal tunnel, and then have us work under fear on top of this? We are better than this.

So I say, boldly put in whatever and how many ever skip markers are appropriate. If it is done correctly, they can never come back at you.

How can the Q make rules about skip markers? They're based on what the dictators mumble at us. The worse the mumble (or accent), the more skip markers we have to leave. Same thing for blanks.

They seem to think we DELIBERATELY leave skip markers and blanks. They must not realize that these markers and blanks represent time lost we spend on repeat listening and research looking for words.

Not to mention time lost on having to replay the file to get the audio slider from leaving huge whited-out areas of text. Half of my ASR reports come up with these huge blanks that look like I haven't listened to them! Sometimes I've had to relisten to the entire file at least 3 times to correct this problem (after I've made the initial edits and proofed)...and even then sometimes it still doesn't.

I took an ASR class about 5 or so months ago and was told that you only put a skip marker over words that would not normally be in a report, like a conversation taken place in the background. All other words that just come up wrong (and I know this is extensive), you should just delete.

It takes less time also to highlight and type over with the correct words than hightlight, place skip marker and then type the right words.

I would ask CCM if you can take a refresher class, and if you have not had one in a year, I believe it is paid for. There might be some short cuts you could use also.

What if that happens consistently throughout the report; let's say more than 5 times? We can't help it if the person dictating the report makes us leave more than 5. Personally, I am not sure a refresher course would help in that case.......

If you get the same doctor over and over and the background conversations come into the ASR, and you have to use more than 5, I am sure this would not be an issue, but it will be an issue if you are just covering any wrong words with a skip marker.

I have 1 doctor that spells everything... ASR puts in the words and then S P E L L S it all out. That would require a marker, and yes, I have put in more than 5 on his repors, and I have never gotten an e-mail or phone call about it, and I have been doing ASR for 4 years.

I am pretty sure you would only get an e-mail if you were not doing it properly.

We will get points taken off on audit if we just highlight and delete 5 words or more in a row.

except for what is dictated at the start with patient name, etc., and have heard nothing about it. Makes it much easier to do ASR now for me and much faster. Stupid system that does not learn anyway so skip markers a waste of our time.

After about 9 months of attempting to teach ASR, I came to the realization that it can't learn a thing from me. I just delete and type. Not a word from anyone.

I quit using them something last year. Why bother, it doesn't learn anyway, plus much faster to just delete!

2-1/2 to 3 yrs ago. When the dictator is hemming/hawing or going over something simple I already read through (being a fast reader really helps) I'm in another section of the report numbering meds or cleaning up something else until the doc says something I need to listen to. Have white patches on my whatever too. Get through my audits with flying colors. The ASR voice and text still match up. I couldn't make a dime doing it any other way. Haven't heard anything from anybody about it.

What are "skip markers?" I only know "markers." I do not work for MQ. - nm

I'm fairly new to MQ and learning the skip marker thing.  Could someone please tell me, are we suppose to remove the markers that say <non-s>?  I've had so many reports that are FILLED with these markers.  Or, are they suppose to stay in? I'd appreciate input/answers.  Thank you, Happy. ...

Some Thoughts On The S Markers

I was wondering why in the new 6.1 version, it takes more words to get an automatic S marker inserter (highlighting words then using the delete button).  I used to have to count the words and if 5 or more, used to place the S marker.  I wonder why now the difference.  Do you think the Q keeps track of how many S markers are generally given in our documents as their ASR system is developed, and now wants to give themselves another pat on the back, exclaiming how their stellar ASR s ...

How Do We Check/uncheck QA Markers?

My first report of the day was an ESL (typical for a Sunday when that's all I get).  I had QA markers, but when I went to send the document, the QA box was already checked and I couldn't uncheck it.  I didn't think another set of ears would have heard what I couldn't.  Obviously this report went on to QA as I was given no choice.  Aren't we supposed to have a choice to check that box starting today? ...

I had a few extremely difficult ESL dictations tonight and reluctantly sent them to QA, only to get them back with some of my QA markers listened to, and others left in the report.  I thought they had to listen to ALL of the QA markers and either correct them or blank them?  I thought we weren't supposed to send reports with QA markers in to the hospitals?  Is this just the MME not doing their job, or something else? When a doctor uses markers at the beginning of each patient's dication in a recording, how can I access those in Express Scribe when listening to it? I have e-mailed the company twice, and the first reply was totally unrelated to my question. There was no reply to the second query. I am currently using the free dl version of ExpressScribe but would glady upgrade to the $30 version if I was sure I could access the markers. Okay so I am going round and round with this.  I understand that MQ will be using weekly figures to determine the submission rate.  I understand how the submission rate is calculated but what I cannot figure out is how they determine what lines to pay the reduced amount on.  My super's comment was that you will be paid the rate on everything after the 10% (3-14 - 5-1) or 5% (after 5-1) that is sent to QA.  So, maybe one of you posters out there, who is not visual ...
