this doctor needs to retire, scary! - hmm

I have this doctor I've been working with for a few years through a company, who is very old. He frequently loses his place while dictating and often doesn't make sense.

Today has been bad already. One report he dictated 3 sentences that did not make sense, nor was it clear what he was really trying to say. Another report he dictated the exam, paused, started to dictate exam and then realized he already did, started with the impression, dictated #1, then as if he was just continuing with impression but was dictating medications, then finished with the plan as usual.

This just scares me that he is still practicing medicine. He can't keep his thoughts straight. I wonder if anyone who works in his office notices these things? This can't be okay.

I'm sure they notice, but I doubt they would do anything. They probably help as much as they can, but ultimately, if it is his own office, he retires and they lose their jobs. We had a doctor at a hospital I use to work for that it took forever for him to retire. Poor guy had health problems. Would have to be out extended periods of time. It's really sad. You know they give so many of their years to medicine and their practice and patients and just have a hard time letting it go.

I feel bad because most of the time when he gets lost in his dictation or can't get out what he is trying to say, he sounds like he is a little frustrated. He will sometimes just stop what he is saying mid sentence and move on.

I type a million young and middle aged ones - who do the same.

I think they are all too busy and trying to juggle too many balls in the name of multitasking. I think they are all scary.

Typed one the other day by a young ESL...she couldn't pronounce 1 med. She could not make a sentence. In the end, she started with impression #1, then impression #2, then impression #9 and gave me 7 more impression after that all starting with #9. I don't think she knows any Arabic numbers other than 1, 2 and 9. I shiver to think of the lab values that were wrong but could not be checked from prior reports.

it probably is time to retire.

I had one doc who would dictate between 11 p.m. and 1 a.m. and he would constantly lose his place and even fall asleep while dictating. Then he would be doing surgeries at 8 a.m. (or earlier) the next morning. Made me sure I never wanted to go to that doc!

A friend of mine was just fired from MQ with no warning whatsoever! Apparently she had CC'd the wrong doctor (very similar-sounding names, ESL dictator). I know this is a major HIPAA violation, but everyone makes mistakes...we are only human. My friend always had excellent QA audits above 98%, 15+ years as MT, no other issues with her work. doctor clearly states "a fair amount of adhesions;" ASR transcribed "some adhesions." That is so bizarre to me and if I wasn't really LISTENING and was just flying through proofreading, I would have missed it. Rambles on, makes no sense throughout the dictation at all, does not make complete sentences, dictates on a patient in the WRONG chart constantly, follows no format, which is required by YOU, cuts himself off but yet when you can understand him, he has a "bright" moment and makes sense is an absolute joy to transcribe???? I got in trouble today because I put a wrong patient. The doctor dictated a billing number, name, and MRN. The MRN was had a different patient, but the billing number and the patient name matched the ADT screen. Not to mention the fact that he kept using "she" in the dictation and the MRN number matched a "he".
