Replacing name with "Patient" - Horski

Is it an industry standard to not use patients' names in the documents even if the dictator dictates it that way, but replace it with "patient"?  Wondering if this is per policy of facility you work at or the industry standard.  Any help would be much appreciated.  Thanks.

facilities want the patient's name in the report if it is dictated. Check with the facility.

Where can I get the standards from JCAH? I am trying to find it, but it is vast. I believe you, but need to provide documentation of that to my boss. Thanks.

Do you have the Book of Style Guide? There is a whole section in there about confidentiality.

I am very familiar with BOS, but was more interested in Joint Commission. You say it has been like that for years. My question I guess really should have been "How do you know this?" I believe that you do know it, but I would like to find where these items are so that I can go to the resource and not have to ask here each time. I search the site and really do not get anywhere. If you can tell me where on their site to find these things I would be very grateful! Thanks!

I have been on accounts where it is the client's preference to always use "the patient," even though some of the docs would still dictate the name in the report (we still had to change it to "patient").

Alot of clinic accounts do not care, just depends.

I contract with is replacing Dictaphone and transcription with Powerscribe and editing.  I have been presented with an extensive contract which will supersede the transcription agreement I have had for 10+ years.  Aside from the disparity in pay and expectation to edit STATS for free, I have serious concerns about signing a contract that states I have full legal liability for the edited report and that I have to carry Workers Comp and 1-3 Million $ E&O and general liability insurance ...

If the client profile makes no mention of patient name (yes or no) being left in the report, account does not specify verbatim, is the ADHI/JCAHO directive to type the patient's name or replace with "the patient?"  Just checkin'.  Am paranoid about everything with the audits these days.  ...

I swear to all that is holy, this is what this less-than-stellar PA just dictated:   "The patient IS a high-risk gestational trophoblastic disease..."  I went back and listened thinking I must have heard wrong and she said patient HAS ... Nope.  She said IS.  This is the same PA who dictated a creatinine of 92 for someone.   That sound you hear is me hitting my head on the desk. ...

Just FYI....I applied to PF about 3-4 months ago. Took the transcription test, thought I did great, never heard a word. Today out of the blue, got a call that I had done great on testing and they would like to offer me a position.  I said thanks but no thanks since I'm no longer looking for a job.
