New job - hundreds of new doctors for all worktypes - MT with new job

I am an experienced transcriptionist and used to ESL and all work types.  I am feeling a bit overwhelmed after being told I am going to be on a hospital account and will have to type 100s of doctors in all work types. 

Is that really as scary as it seems to me?  I can imagine going all day w/o transcribing the same doctor twice.  Yet I'm supposed to meet production criteria. 

If anyone has had an experience like this and ended up thriving, please share your wisdom.

Large facilities often have large physician lists but - many are not regular staff.

I can only speak in generalities, but I've worked enormous facilities (university hospitals with all specialty clinics, etc.) and found that there were many physicians who had staff privileges but would usually practice elsewhere and would only refer patients into the U. hospital when they had challenging issues, needed to be on a research protocol, etc. Hence you never heard from these doctors.

The Pareto Rule would predict that 20% of the doctors will generate 80% of the dictation. I'm sure this isn't precisely true, but some variation of the rule does apply. Perhaps 40% of the doctors generate 60% of the dictation, or something like that.

In any event, the most difficulties seemed to revolve not around the staff physicians, but the ever-changing panorama of interns and residents, and they do dictate quite a bit. If yours is a teaching hospital, that's where you'll find the challenges, because many of them:

1. Are completely new to dictating, disorganized, rambling.
2. Use odd abbreviations and acronyms that will baffle you. This will be their own shorthand in their notes that they don't realize should not be dictated.
3. Are experienced dictators, but rushed off their feet, fagged out from working 16-hour shifts, and dictation is ragged and awful - if they don't fall asleep in the middle of a dictation or start mumbling nonsense from a waking dream.

On the other side of the coin, if you're a highly experienced MT, dictation is pretty much dictation and you should be able to adapt.

Oh, yes - the other thing: Huge facilities, especially those with lots of different clinics, tend to have lots of different work types and lots of "site specifics". This will be another significant challenge until you get some time under your belt.

I can't say it's not more difficult than a typical acute-care hospital, but I don't think you need be overly anxious about it either.

I transcribe for a large teaching facility associated with a university, and there are several associated outpatient clinics in 2 states as well that send dictation, but it really does seem like only 20% or 30% of the dictators on our huge list dictate. We have a lot of residents dictating that are learning how to dictate, and that can be frustrating, especially the ESL residents who have a very hard time with the language and sentence structure of American English. It is not too bad, though. Right now in July and August, we seem to get a bunch of new residents, but then it slows down for the rest of the year. I really do hear a lot of the same voices each day, and I like the variety of specialties. Keeps it interesting.

you just take one report at a time, just like one step at a time, and you'll get there. You'll likely have several doctors that dictate frequently, so its not like you'll never have same doctor twice. If possible make copies of difficult dictators and put them alphabetically in a binder. Maybe even highlight phrases you had a difficult time with. I'm betting you'll do just fine!

out names will slow you down as much as everything else combined with you're first starting a large account, so don't let them. Enter people who dictate for others tagged with the name(s) of whoever they work with so you can identify them that way.

Doctors Who Cut Themselves Off

Doesn't it drive you nuts when a doctor consistently cuts himself off when he dictates?  I sometimes get half words, the last couple letters of words, places where I haves to leave blanks because they say something like:  EKG showed (silence) rhythm.  What kind of regular?  Regular rate and....?  Arrrrggggghhhh! encounters, are they required to have a short course on proper dictation procedure? I would think, being that this medical record is sooo freaking important that we are drawn and quartered if not perfect, that these professionals who generate these reports would also be schooled about proper pronunciation, enunciation, etc. before they throw it out there for us to perfect. To the poster who posted below looking for information on TTD.  They don't have their own accounts.  They work for Superior Global and maybe others.  Superior pays them to find ICs.  They probably pay them 4 cpl and then TTD pays you a cruddy 3 cpl to edit VR reports.  They don't offer direct deposit because Superior is paying them and then they pay you, which is why you have to wait a month for your check.   I would never work for a company that is just bascially an agency hir ...

Why, oh why, oh why can't the students hit the PAUSE button when they are dictating to gather their thoughts?  Why must I sit through listening to page turning, and "thinking" noises?  Meanwhile, time is money in this job ... and I really am getting tired of this!  My main account is a large teaching hospital, and so I get alot of students, residents, fellows ... etc.  <grrrrrrrrr> OK .. that they are dictating for ASR and how to do it, punctuation and all. They need to take some responsibility for this. If they would dictate for ASR, ASR output would likely be better. I base this on the doctors who do dictate everything, including punctuation and the fact the output is good for this doctors. Although, these doctors do tend to speak in a more understandable manner. My girlfriend told her internist about our plight and the doctor said that they were actually forbidden to use a transcription service and to do it themselves instead.  She hates it and would rather have a transcriptionist do it because they are so good at what they do!   I don't understand why they would be forbidden to use a transcriptionist.
