To find out if I can out some of these hospitals who send medical records over the internet or FTP. People need to know that their records are wide open to possible breaches. FYI

We should not stop at the hospitals. We should out anyone who is processing medical records over the internet. We don't need to do it that way when we can "go back to basics" and do the old way.

So, basically what you are saying mte is that you want to out all the facilities that electronically transfer files which would cause all at-home MTs to lose their jobs?????? Are you serious? How selfish are you!? There is a reason why HIPAA is in place, encryption is in place, confidentiality agreements are in place, etc. etc. and etc.!!! If transfering files over the internet or FTP was 100% dangerous, HIPAA would not allow it nor would HIPAA even exist. If there was any doubt whatsoever that it was unsafe, it would not be allowed!!!!

What if you get run over by an 18 wheeler tomorrow? It is ridiculous and unbelievable that grown people are gnawing at their toenails with such crapola. I do not care about what ifs, I am not scared, I live for this moment, might not have the next. What if my job gives out, what if, what if. I do not CARE about such talk.

I TOTALLY AGREE! I mean wake up and smell the coffee people! One day and probably sometime in the near future, the MT field will disappear or MTs will be working for 2 cents a line or less. It has happened in other professions where technology has replaced or evolved different jobs and it WILL happen in MT. The Internet is used for online banking for chrissakes and other stuff . . . someone's medical records are no more important than someone's financial information. Get real, quit worrying about it because it will happen - MT jobs will disappear. When that does happen, just hope and pray that the person or organization who makes that decision is ethical enough to not group or stereotype you into a pigeon hole and you can move into another career seamlessly.

MTs live for now (what is even there) because in 5 years I bet it won't be there.

That's not going to happen. Hospitals can't afford it. MTs that live out in the boonies can't afford to pick up and move wherever in the country there happens to be an in-house job, especially if its only part-time. If hospitals were forced to keep the work in-house they'd buy a front-end SR program, which docs would edit themselves, and eliminate the MTs entirely. Even so, the docs that are used to dictating from home would scream bloody murder at having to be physically on-site to dictate. Docs that can access PACS from home when staff calls to alert them of a patient crisis aren't going to give up all that patient information and waste the time to drive there and physically look at the in-house computer before prescribing what the patient needs.

You're having an anti-technology fantasy. Wake up. They might regulate, firewall and password everyone to a ridiculous extent soon, but they're not just going to toss the technology, which is too convenient for too many.

I spoke with a rep from CMS under which HIPAA violations are investigated. I was told that they would not do anything until and unless there was a breach. So they're not in the business of regulation, just enforcement.

Has anyone even heard of SSL encryption? That is probably old technology by now! I am not an IT person, but everyone under the sun uses the Internet (web based) with some type of high-tech encryption to process and send files. I think that healthcare just needs to get with the program, buy some new equipment, hire some people in IT who really give a dam_ and not just want to maintain the status quo, and stop with the antiquated ways already.

If we end up with nationalized healthcare for everyone and they go strictly with electronic medical records, you really are going to have to wonder exactly WHO is going to know your personal medical history. I personally don't think it's one bit of the government's business. How many people see therapists? Do you want your elected officials to know what you tell your therapist?

Gladly, I don't do drugs and I don't drink alcohol. I also plan on quitting smoking by my 43rd birthday this Sunday. I want to be healthy and live a long life.

What kinda frightens me, I mean - I know it's against the law and everything that some of the patients we do reports on use illicit drugs, but as it is now, they are able to tell their doctors about it which is very important because you really do need to be totally honest with your doctor. The doctor then dictates that info to be transcribed into the report that goes into the medical record and can be referenced by the PCP or maybe a specialist in the future. Now, with people knowing that government officials can get into our medical records, how many of them are going to start being dishonest about their health history, drug use, delusions, suicidal ideations, etc., and how many times will that end up being detrimental to their health? They've talked about higher premiums for smokers, people who are overweight, etc. I don't know if that will happen or not but hey - if you smoked and could barely afford what the government charged you for their basic health plan on top of the raised taxes for same, and you simply could not afford the extra money it would cost for said plan with an extra premium because you are a smoker, wouldn't you be tempted to lie to your doctor about it? I wonder if fewer people will seek things like 30 day rehabs, for fear of the government finding out they were obtaining illegal drugs. And will drug busts start resulting because now the gov't will know the names and the rest of the info on people who admit to using illegal drugs? As far as rehabs, I know there's a high relapse rate of addicts and alcoholics but you know what? Even though almost no addict or alcoholic gets and stays clean on the first try, a fair amount of them do eventually get it on subsequent tries. Some on the second, some on the 20th. But a fair amount *do* eventually get it and as long as they pick themselves up and go back to those AA/NA meetings after a relapse and try again, that's all that matters. But some may fear entering a rehab because of the info that will be released to the gov't. A lot of people have been helped by rehabs.

And who is going to define overweight? Are we talking morbidly obese? 20 lbs? 60 lbs? How many people, especially a lot of diabetics who are overweight, are going to avoid doctor's office visits because they're afraid of that raise in their gov't plan premium because their weight will have to be submitted to the gov't? Now they're more likely to have more problems with their diabetes and the complications. And the most serious question of all - exactly how is the government going to know and/or find out who is a smoker? Who is overweight? Who uses illicit drugs? I don't know about you folks, but I've read the census questions for this year and I'm not answering my door for them, or if I do, I'm closing it again. They can know how many people live here and that's it. And if I find out it's ACORN running it, they had better run for their lives if they step on my property.

I've wanted to quit smoking for years. I managed to quit cold turkey for six months a few years back, then my FIL died. Too easy to pick back up. I'm quitting now out of sheer anger at all the tax hikes that are already in place and that are coming. I refuse to give the gov't any more of my money than I have to.

As far as my reports being typed overseas, my only fear would be inaccuracy. I really don't care if someone I'm never going to meet types my medical history. I have depression and panic/anxiety attacks with the panic attacks being pretty bad. And I have a very mild case of OCD but it doesn't affect me that often. That's probably my most embarrassing things. Now you guys know too. So what? Same with those new body scanners they want at airports. Good grief. I've had a hysterectomy after which the mesh came thru the vaginal cuff and I had to have vaginal cuff surgery 17 months later to cover the mesh, and now I suffer from chronic dyspareunia and the doctor just blows that off but I'm just pointing out how exposed I've been. I've also had 2 diagnostic laparoscopies, several fertility work ups and procedures, and a lap chole. My body has been exposed to doctors, nurses, techs, etc. I don't care if some person at an airport that there's probably about 1 chance in a billion I'll ever see again watches me thru a body scanner while I'm fully clothed. I'd probably even be a smart alect and wear really skimpy underwear just for grins and giggles. I try to have fun in life when I can. I'd rather be less modest and more safe.

Sorry. I'm going thru my monthly insomnia phase and that's when I'm at my worst with rambling. I'm shut up now.

I truly hope everyone is having a great and productive day! It's darned cold here in Ohio. Tomorrow will be in 30's and it will be a heatwave. LOL!

Yes, with all medical records linked together, it just gets infinitely easier for those with a "need to know" (aka those with the power to deny claims and coverage) to dig up any dirt possible to stamp DENIED on it. Even if you get on a health kick, stop smoking, lose weight, no longer need insulin or other drugs due to lifestyle changes...what's to say the powers that be won't still penalize you for what your history used to be? I learned a long time ago that seeking help for a substance problem or a personality issue can and will be used against you in a divorce...so why not by your insurance company? They say all's fair in love and war - and they LOVE money and will fight to keep from paying it to providers on anyone's behalf, "necessary" or not.

I'm too tired right now to write a lengthy response, but just want to say I definitely agree with you. I keep hearing MT says they are worried about their jobs going overseas, but I have never actually read a news article about an MTs job having gone. I'm sure it's happened, but I've never read about it.

But I agree....the govt is what we really need to be worried about.

Short Cut For List Punctuation In A Drug List? (Fluency)

Is there is quick way to add commas between items in a list? Of course, using Ctl K in front a word will change the punctuation, but it will also change a capitalized drug to lower case, and you will still have to get to the beginning of the next drug which will require 2 Ctl arrows and backspace. Ctl arrow takes you to the beginning of the next word, but you will have to backspace to put in the comma. Adding punctuation to lists is so time consuming! ...

If you are filling out a job application and it asks you what your last pay was at your last job, what do you put if Nuance is your last job? How do you write that? Just put 7 cpl? If you put starting pay and then ending pay, isnt that going to look terrible on you? Like they lowered YOUR pay so much as compared to when you started. I'm having trouble with the format of this psych consult. Under the DIAGNOSIS heading, the dictator lists the Axes as follows: Axis I:  Adjustment disorder with anxiety, moderate in degree. Then he says, "new paragraph" but continues on with Axis I to say "Psychogenic amnesia." Since we have a (lengthy) Do Not Use list, I think the doctors should be versed in proper dictating form with a Do Not Dictate list to include "next", "next number", "new line", "skip line", "and", and "uh, uh, uh" and all those other little irritating words that ASR loves!  Charge them 3 cents MORE per line of dictation if they can't maintain 98% with no blanks sent back to them!!!!!!!! ...
