There is a limited number of people in India and The Philippines who speak English well enough to do this kind of work (and fluent English speakers aren't all deciding to go into Medical Transcription, because it doesn't pay as well as other English-fluency jobs). In other words, the shortage is world-wide, and American transcriptionists are the easiest, best, and most abundant source for this job skill. The need for people who can do this work is going up, and the supply of qualified English-fluent transcriptionists is going down. This may be the dark ages of transcription, but I think that there will be something brighter in the future.
