that companies weren't paying for templates, normals and highlighted QT text.
I'd just have to refuse to work on any account that wasn't paying for this, and if it is a company-wide situation where I was doing a lot of this type of work, I'd quit.
You're essentially working for free in some cases, especially when you're talking about inserting macros/normals and having to choose these items from a long list of available options.
It takes time to do this, and it also takes time to choose templates. If clients don't want to pay for this then they should just have their office people do the reports. It's ridiculous what's being expected of MTs now to save a client who doesn't want to pay what the work is worth.
If it's uncompensable, let their own office staff do it!
MTs should NOT be doing this for free under any circumstances.
