If an account that used to have lots of work is suddenly running out all the time, there is a typical reason: Nuance is probably giving (large) bits and pieces of that account away to other transcription companies.

Here is the secret: Look in the pend list of your account. Are there a bunch of different-but-same pend reasons, like ABC_Blanks, GGH_Blanks, PDQ_Blanks, and XYZ_Blanks... of which you only use one of those three-letter combinations? Those other 123_Blanks are for other MTSOs that have access to your account. They likely are doing all the work you used to get.

Go to Google and use those three letters to figure out what other transcription companies might also be working on your account. Get in touch with them, ask them if they have such-and-such an account, and if they do, let them know that you are from so-and-so MTSO, that you are now always running out of work, but that you are fully trained and experienced on that account and ask if they would be interested in hiring you.

If you find the correct MTSO who has been given all the work by Nuance that you are no longer seeing, you can bet that that MTSO will snatch up a fully-trained and account-experienced transcriptionist in an instant. I'm not saying the MTSO will be any good, or the pay rates will be the same... but that's for you to deal with. I'm just letting you know where to find your lost work.
