The newer trend is to do as poster has described so well above. You will still find many companies and clinics that don’t expect them to be capitalized at all. The reason is because there is a sort of unspoken “the/his/her” in medical, and especially pediatrics, so when they say “and child is brought in by mom for...” they are not saying “Mikey is brought in by Jane for” but “the child is brought in by his mom for...” They wouldn’t expect child or baby to be capitalized either. When they say “and baby weighs 8 pounds,” that is not capitalized, but “and Baby Boy Jones weighs 8 pounds” is. So the capitalization of mom and dad is not always done. My MTSO has moved in that direction too, but I also type for a pediatric clinic and they think the capitals look odd.
