How in God's name do some of these people get a medical degree ? I've got an NP dictating "the patient was encouraged with tobacco SENSATION" When you get dictator after dictator saying crap like this, you just want to reach through the computer and smack them right in the mouth. Of course, I've got to change it so it's correct but SOMEONE should make this fool aware of what she is saying. Just like the one doctor I have that consistently says "review of SYMPTOMS"
