I signed an "anonymous" survey one time and found out later that the exact answers I used were put into a conference call discussion and they knew exactly who wrote that because it was also discussed in my yearly review.
Electronic tracking, people. There are electronic footprints that track every single keystroke you make. It's not paranoid, it's not a myth. It is real.
Yes, I was told this by several IT people over the years, the same thing, every single keystroke is tracked and recorded, and if you're somewhere you dont belong, even by accident, flags go up to alert the IT staff and you're "Spoken To" about it.
