I have noticed lately that every time it "hears" a medication ending in statin, it is now outputting simvastatin no matter what statin was dictated!
Add that to the rest of output not as dictated. The patient has fever - you get The patient has fever, chills. Too many times, when with is dictated, output is without, or if without is dictated, output is with, consistently changes a negative to a positive, or positive to negative. Etc., etc., ect.....
Rant for the moment.
