Strict verbatim account:  "Patient has cough, wheezing, short of breath which is nonproductive."

There's no way to win.  The phrase about nonproductive is out of position, "short" of breath s/b "shortness" because it should be an object rather than an adverb phrase, etc.

My blood pressure is rising, depression increasing, hair is stand on end 200/120.
