I would have typed a DOS heading in the report and put a blank under it. Otherwise, it just looks like you forgot to include the DOS. Leaving a blank or a QA note (which you couldn't do) would have stopped it client side and had them figure it out. Leaving the blank would have also covered your bum against the auditors.
