I have been part of a health care sharing plan since 1994. Yes, part of it is having my pastor sign a once-yearly form stating, among other things, that I attend church regularly. By that I think it means "once a week." I have never read in any of the paperwork that more than once a week is needed to meet that requirement. The reason for that might be pretty clear to some but a stumbling block for others. Any other questions, e-mail me and I would be glad to help.
