Critical error for "not removing the question mark" - FrustratedMT

I am heading for performance management because apparently I "don't search the database and am submitting ADT with the question mark attached." This is the 4th time I've gotten this in an audit and it's extremely false. I know for a fact I'm searching because I do it every time, no matter what. I have a sticky note on my computer to remind me. I emailed back and told her that I know for a fact I am submitting correctly, so we will see how long I last.
